Statement of Work
Background:
The NY Harbor, Manhattan Medical is seeking a project to replace Laboratory Terrace roofing on 18N. Repair 17N ceilings in room 17144N Day room, Room 17100N Patient dining room, and room 17104 Bed 5/6.
Description of Work:
Provide service to replace the laboratory y terrace roofing. for the Veteran s Affairs Medical Center, 423 East 23rd Street, New York, NY 10010 in accordance with VA specifications at http://www.cfm.va.gov/til/specs.asp. Work shall meet all VA design guidelines and requirement.
The Contractor shall furnish all labor, materials, tools, supervision, equipment, safety, necessary to install replace the roof. At minimum, the following shall be performed:
The Contractor scope to supply all labor and materials to do the following:
1. REPLACE SMALL ROOF AREA 18 NORTH

-Remove existing roofing materials down to concrete surface.
-Remove existing stucco at ducts 1 from floor level.
- Prepare roofing substrate as per Kemper System manufacture requirements. Apply AC Speed FR fully reinforced roof membrane.
-Traffic Coating, install traffic coating with silica aggregate and seal using Kemper AC gray sealer.
- Stucco, Install polystyrene insulation with floor level weep holes at the duct niche. Apply reinforced base and acrylic stucco finish coating.
Drain Replacement, remove existing drain and replace with Smith 2130 Medium Duty Floor Drain with 12" Round Top Flood Test Perform 24hr Flood Test & Supervision for Duration
2. Investigate and provide report for ceiling leaks in rooms 17100N, 17144N, and 17140N and determine source of leak including plumbing repairs.

3. ROOM 17100N PATIENT DINING ROOM,

-Install plastic zip wall over existing door, setup negative air machine, fire extinguisher, negative air monitor.
-Cap existing sprinkler heads.
-Install temporary lighting.
-Remove and dispose of existing 2x2 anti-ligature/tamper proof light fixtures.
-Remove and dispose of existing (1) 2x2 anti-ligature/tamper proof ceiling diffuser, (2) 25"x6" anti-ligature/tamper proof ceiling diffusers. Demolish and dispose of existing damaged plaster ceiling.
-Install new 16-gauge ceiling framing and Type X 5/8" gypsum board.
Install (1) new 2x2 anti-ligature/tamper proof light fixture and (2) 25"x6" anti-ligature ceiling diffusers.

-Install (3) new 2x2 anti-ligature/tamper proof and anti-ligature ceiling access panels

4. ROOM 17144N DAY ROOM

-Install plastic zip wall over existing door, setup negative air machine, fire extinguisher, negative air monitor
-Cap existing sprinkler heads
-Install temporary lighting
-Remove and dispose of existing 2x2 anti-ligature/tamper proof light fixtures
-Remove and dispose of existing (1) 49"x6" anti-ligature/tamper proof ceiling diffuser, (2) 25"x6" anti-ligature/tamper proof ceiling diffusers.
-Demolish and dispose of existing damaged plaster ceiling area
-Demolish and dispose of existing damaged plaster ceiling area
-Install new 16-gauge ceiling framing and Type X 5/8" gypsum board
-Install (1) new 49"x6" anti-ligature/tamper proof light fixture and (2) 25"x6" anti-ligature ceiling diffusers
-Install new anti-ligature/tamper proof sprinkler heads
-Install (3) new 2x2 anti-ligature/tamper proof and anti-ligature ceiling access panels

- Clean-up, handover
5. ROOM 17104 BEDS 5/6

-Install plastic zip wall over existing door, setup negative air machine, fire extinguisher, negative air monitor
-Cap existing sprinkler heads
-Install temporary lighting
-Remove and dispose of existing (4) 2x2 and (1) 2x4 anti-ligature/tamper proof light fixtures.
-Remove and dispose of existing (2) 25"x7" anti-ligature/tamper proof ceiling diffusers and (1) 2x2 anti-ligature/tamper proof ceiling diffuser.
-Demolish and dispose of existing damaged plaster ceiling area
-Demolish and dispose of damaged wall lath and plaster
-Install new 16-gauge ceiling framing and Type X 5/8" gypsum board
-Repair wall with new lath and plaster
-Install new (2) 25"x7" anti-ligature/tamper proof ceiling diffusers and (1) 2x2 anti-ligature/tamper proof ceiling diffuser
-Install new anti-ligature/tamper proof sprinkler heads
-Install (1) new 2x2 anti-ligature/tamper proof and anti-ligature ceiling access panel
-Spackle, sand, and paint new ceiling and wall area

General Requirement:
The Contractor shall furnish all labor, materials, tools, supervision, equipment, safety, necessary to perform the repair. At minimum, the following shall be performed:
1. The Contractor shall take all necessary precautions and safety measure to protect all patients and end users from all repair related activities.

1. The Contractor shall test and verify all repairs to ensure proper operation.

2. The Contractor must have OSHA training, and specialized OSHA training. Lock-out/Tag-out is required prior to commencement of work. The Contracting Officer or his/her designee will notify the contractor of any noncompliance and the action to be taken. The Contractor shall, after receipt of such notice, immediately correct the conditions to which attention has been directed. Such notice, when served on the Contractor or his/her representative at the site of the work, shall be deemed sufficient for the purpose aforesaid. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all and any part of the work and hold the Contractor in default.

3. All work shall be done in accordance with NFPA, Life Safety, VA Standards and Regulations, and OSHA regulations. In the performance of all work, the workers must be incompliance with safety regulations governing worker safety (OSHA: 29 CFR 1926 Safety and Health Regulations for Construction; NFPA, Life Safety Codes, etc.).

4. The Contractor shall follow all Infection Control measures and Interim Life Safety measures. All work performed will be in an operational healthcare facility.

5. All work shall be done in compliance with all VA Policy and requirements.

6. The Contractor shall provide daily log, certified payroll, and any required submittals. The Engineering Logbook shall be signed each day the Contractor perform work. The Contractor shall complete VA Privacy Training and VA Badge Requirement.

7. Contractor shall account for any additional expenses for afterhours/weekend work.

8. The Contractor shall guarantee that all work performed will be free from all defects in workmanship and materials and that all installation and repair will provide the capacities and characteristics specified. Contractor shall guarantee that if, during a period of one year from the date of the certificate of completion and acceptance of the work, any such defects shall be repaired by the contractor at his own cost.

9. It is the responsibility of the contractor to verify all quantities, field conditions and measurements prior to starting work. The contractor shall notify the CO/COR and VA Engineering Team of any differing site conditions prior to starting.

10. The Contractor shall provide an English-Speaking working supervisor on-site at all times when work is being performed and for the duration of this project. Supervisor must have a minimum of five (5) years of experience as a supervisor on five (5) projects like this.

11. The Contractor shall provide his own storage for materials. The VA is not responsible for loss of tools, materials, equipment, etc.

12. The Contractor shall post visible required warning signs and barriers prior to beginning any work.

13. The Contractor shall be responsible for any damage caused by his employees. All damages shall be repaired and/or replaced at no additional cost to the government.

14. The Contractor shall be responsible for the proper and safe removal and disposal of all debris.

15. The Contractor shall perform cleanup at any time when work area becomes cluttered and/or restricts traffic. At minimum, the Contractor shall clean work area at the end of each work shift.

16. The Contractor must coordinate and schedule all work with the VA.

17. The C&A requirements do not apply, and that a Security Accreditation Package is not required.

18. The Contractor is responsible for completing the VA Privacy and Information Security Awareness training.

VA Patient Privacy and Network Infrastructure
None of the equipment listed within this procurement package will access the VA Network or store any sensitive patient information. Any contractors that will come on site for install will not need access to VA network. All contractors must comply with VA regulation and complete the following training: VA Privacy Training for Personnel without Access to VA Computer Systems or Direct Access or use to VA Sensitive Information.
Reference the attached Handbook 6500.6 Appendix A - Checklist for Information Security in the initiation phase of acquisitions.

Privacy Training:
VA Privacy Training for Personnel without Access to VA Computer Systems or Direct Access to or the Use of VA Sensitive Information
All contractor employees and subcontractors participating under this contract are required to complete the VA's Privacy training requirement - VA Privacy Training for Personnel without Access to VA Computer Systems or Direct Access to or the Use of VA Sensitive Information Contractors must provide certifications of completion to the COR during each year of the contract. Training expires 365 days after the training is taken. This requirement is in addition to any other training that may be required of the contractor and subcontractor(s). The COR for the contract is responsible for ensuring the contractor takes the training and to follow up with the annual requirements.
