Description
1) The subject of the contract is to provide the Contracting Authority with periodic collection, transport and neutralisation of medical and hazardous waste produced in the J. Gromkowski Provincial Specialist Hospital in Wrocław at ul. Koszarowa 5.2) Estimated quantity of medical waste to be collected, transported and disposed of during the term of the contract with the following codes: - 18 01 01 01 in the amount of 0.01 Mg,- 18 01 02* in the amount of 1 Mg,- 18 01 03* in the amount of 300 Mg,- 18 01 04 in the amount of 200 Mg,- 18 01 06* in the amount of 0.05 Mg- 18 01 07 in the amount of 0.08 Mg- 18 01 09 in the amount of 0.06 Mg- 18 01 10* in the amount of 0.001 Mg The Ordering Party reserves that the quantities of medical waste indicated are the quantities of estimated medical waste. The amount of waste depends on the number of patients and the number of procedures performed.3) The Ordering Party expects:- weighing, loading of waste in the presence of a designated representative of the Ordering Party - collecting medical waste from two warehouses located in the Hospital at 5 Koszarowa Street, - construction A-2, construction A-3 for 3 days a week, i.e. on Monday, Wednesday, Friday at 7:00-14:00. In case of accumulation of days off, the Contractor guarantees collection of medical waste, so that its storage period on the Hospital premises does not exceed 72 hours. Waste disposal shall be carried out by a specialized means of transport of the Contractor in compliance with the regulations applicable to the transport of dangerous goods. Contractor's vehicle with a capacity of up to 10 tons - due to difficulties in entering the hospital area and later manoeuvring at waste warehouses, - substitution of electronic scales in the Ordering Party's waste warehouses for the commission weighing of waste for the duration of the contract. 2 weights - bud: A2, A3 with dimensions 1m x1m, crane min. 200 kg, each time waste is weighed in the presence of the Ordering Party's employee, - holding a permit for medical waste management activities, i.e. collection, neutralisation and, with regard to waste transport, submission of a document confirming entry in an appropriate register and assignment of a registration number in accordance with the provisions of the Act of 14 December 2012 on waste (Journal of Laws 2018, Journal of Laws of 2012, No. 152, item 259, as amended).Each time the Contractor confirms the quantitative takeover of waste issued by the Contractor at each collection and weighing of waste in the so-called "Waste Transfer Sheet", - taking responsibility for the provision of this service pursuant to Article 27 (3) of the Act on waste (i.e. Journal of Laws, Dz.U. of 2009, No. 153, item 259, as amended), - each time the Contractor confirms the quantitative takeover of waste issued by the Contractor at each collection and weighing of waste in the so-called "Waste Transfer Sheet". observance of the proximity principle referred to in Article 20 of the Act on waste, - disposal of medical and veterinary waste collected from the Contracting Authority in the Lower Silesian Voivodship in accordance with Article 21 of the Act on waste. Pursuant to Article 20(3)(2) of the Waste Act, at the same time, pursuant to Article 20(6) of the Waste Act, infectious medical and infectious veterinary waste may be disposed of on the territory of a province other than the one where it was produced, in the closest installation, if there is no installation for disposal of such waste on the territory of a given province or if the existing installations do not have free processing capacity. Medical waste collected from the place of manufacture, i.e. J. Gromkowski Provincial Specialist Hospital, ul. Koszarowa 5, 51-149 Wrocław, POLAND; it will be each time, immediately after receipt, it will be transferred for neutralisation. The Contractor shall indicate in the offer form the name and address of the incineration plant in which medical waste will be disposed of, - due to the lack of marks, the remaining description can be found in the ToR in section 3.
