E.12 INSTRUCTIONS TO OFFEROR S EVALUATION OF QUOTES

Basis for Award. The Government will issue a purchase order to the responsible quoter whose quotation conforming to the solicitation will be most advantageous to the Government.
The following factors shall be used to evaluate quotations:
Technical Ability for contractor to demonstrate technical approach and methods meet industry standards.
Past performance Successful past performance similar in size and scope
Price Most advantageous to the Government

Evaluation Approach. The Government will evaluate quotations using the comparative evaluation process outlined in FAR 13.106-2 (b) (3), where quotations will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation but is not required to do so. Each response must meet the minimum requirements of the solicitation. The evaluation will consider the following:
Technical: The quotation will be evaluated to the extent to which it can meet and/or exceed the Government s requirements as outlined in the solicitation and based on the information requested in the instructions to quoters section of the solicitation, specifically the following:
Did the offeror submit proof of EMFTS License at the time of offer.
Did the offeror submit proof of EMFTS License at the time of offer.
Did the offeror show proof of insurance to perform requirement listed in the PWS.
Did the offeror complete the DoL WD compliance Memorandum (Attachment D.2)
Did the offeror complete FAR 52.219-14 Limitations on Subcontracting (Attachment D.3)
Did the offeror submit the most recent SF 1449, Sign all SF 30 Amendments, as well as complete table in section B(1) to acknowledge all Amendments.
Technical approach which shows (provided in narrative):
The equipment to be used to provide services listed in the PWS.
How the contractor will meet the required schedule.
Reporting of issues and corrective actions.
Contingency Plan: Government will evaluate the offeror s plan to fill requirement when situations arise that create hardship(s) in providing that service to include employee shortage and/or a short term/long term influx in contract utilization. Also, contingency plan will cover hardship(s) of vehicle downtime due to but limited to maintenance or out of commission.

Past Performance: The past performance evaluation will assess the relative risks associated with a quoter s likelihood of success in fulfilling the solicitation s requirements as indicated by the quoter s record of past performance. The past performance evaluation may be based on the contracting officer s knowledge of and previous experience with the supply or service being acquired; Customer surveys, and past performance questionnaire replies; Contractor Performance Assessment Reporting System (CPARS) at http://www.cpars.gov/; or any other reasonable basis.
Past Performance will be limited to past 3 years of evaluations.
Past Performance will be for PCS V226 or equivalent.
Past Performance will be labelled as the following:
A past performance rating of Exceptional is assigned a Substantial Confidence rating.
A past performance rating of Very Good or Satisfactory is assigned a Limited Confidence rating.
A past performance rating is assigned a Neutral Confidence rating if no rating can be found.
A past performance rating of Marginal or unsatisfactory is assigned No Confidence rating.

Price: The Government will evaluate the total price consisting of Base plus four option years. Quoted price shall include all labor, materials, maintenance, technical support, project management and supervision, and support as defined in the PWS that are necessary for the offeror s to perform the required services. Pricing shall be listed on the Price/Cost Schedule found in B.1 of the RFQ.

How to submit offers: All offers shall be submitted via email to robert.bolcavage@va.gov no later than 1:00 PM Eastern standard time on 23 April 2024. Emails shall not exceed 9MB in total size. Inside the body of the email the following information is required:
Contracting Name (Name that will be on the contract if Awarded)
Cage Code and Unique Entity ID number from SAM of that Contractor.
Date of issue of the EMFTS license that is in the name of the Contractor.
Confirm any Social-Economic status and/or business size status of the Contractor.
POC of individual submitting the offer to include email and phone number.

*** Any questions or request for clarification over this RFQ will not be accepted after 4:00 PM Eastern standard time on 15 April 2024. The questions SHALL be identified separately and not in the middle of a paragraph. All questions shall end with a ? . Not identifying a question may result in those questions being overlooked and not answered. Questions will not be reworded to fix spelling or grammar. If multiple questions regarding the same question are received, those questions will be answered once and not individually. ***
Examples of a question and government response format:
Question 1: Is this the format you want questions to be asked for this RFQ?
Response 1: Yes, that is exactly how questions should be written.
Question 2: Questions or Clarifications not in this format may be overlooked, Correct?
Response 2: Yes, it will not be done intentionally, but it may happen.
Question 3: If you find you overlooked a question not in the format requested, will the CO publish a second amendment to answer the missed question?
Response 3: No, if the CO overlooks a question not in the requested format, the CO will not publish a second amendment to answer the question. If the questions are in the proper format and the CO overlooked the question, then yes, the CO will publish an additional amendment to answer the question.
