The subject matter of the procedure and the contract is to select the Economic Operator for the supply of phantoms (mannequins, training models and trainers) for the needs of the Centre for Innovative Medical Education of the Jagiellonian University - Collegium Medicum in Kraków, respectively, for one to nine parts of the contract.1.1 The contract is co-financed by the European Union in connection with the implementation of the project "Education, Innovation, Development. Improving the quality of education through the development of innovative education "From the resources of the European Social Fund under the Operational Programme Knowledge Education Development, agreement no.: POWR.05.03.00-00-0006/15-00.1.2. The scope of the contract includes in particular purchase, transport, packaging, unloading, bringing in and commissioning in the places indicated by the Employer, warranty service as well as provision of warranty services, respectively, with reference to one to nine parts of the contract.1The subject of the contract covers brand new equipment, not used, not exhibited, manufactured not earlier than in 2019 according to the following general list of types and quantities of phantoms (mannequins, models and training trainers) constituting the subject of the contract, respectively, with reference to one to nine parts of the contract:1.3.1 Part 1 of the subject of the contract - Set of appearances - 1 set,1.3.2 Part 2 of the subject of the contract - Rescue dummy - 1 piece,1.3.3 Part 3 of the subject of the contract - Traumatic phantom - 1 piece,1.3.4 Part 4 of the subject of the contract - Adult tube fitting model - 2 pieces,1.3.5 Part 5 of the order - Models: a) Child-only model (A) - 1 piece,b) Child-only model (B) - 1 piece,1.3.6 Part 6 of the order - Trainer - Women's chest tube test (A) - 3 pieces,1.3.7 Part 7 - Trainer - examination of the breast gland of women (B) - 2 pieces,1.3.8 Part 8 - Trainer - Supplementary accesses - 5 pieces,1.3.9 Part 9 - Trainers: a) Trainer - subcutaneous injections - 5 pieces,b) Trainer - Intramuscular injections - 5 pieces.1.4 A detailed description of the subject matter of the contract, specifying the functions, technical parameters and conditions required, or their maximum or minimum ranges or other necessary features, and additionally scoring, is included in Annexes A, B, C, D, E, F, G, H, I to the Proposal Form, which constitute an integral part of the ToR, respectively for one to nine parts of the contract.1.5 The Economic Operator shall ensure safe packaging of the subject of the contract, protecting it against weather conditions, mechanical damage and preventing its incomplete completion.1.6 The Contractor is liable for damage to or loss of the subject of the order until the confirmation of receipt by the Ordering Party.1.7 The delivery will be made on working days, i.e. from Monday to Friday between 9:00 and 13:00. The Contractor undertakes to deliver the subject of the order to the place indicated by the User, i.e. the Medical Education Department of the Medical Department of the Jagiellonian University CM; Św.Łazarza Street 16; 31-530 Kraków.1.8 The Contractor is obliged to notify the Ordering Party of the planned delivery date at least 2 (two) working days in advance. The Ordering Party reserves that if the designated delivery day is a non-working day at Jagiellonian University, then the designated delivery date will be the first working day after the non-working days.1.9 Other provisions concerning the description of the order are included in point 3) of the ToR.
