Description

The subject of the contract is the provision of operation and maintenance services of the following equipment, installations and networks: water-supply, heat (central heating, steam, hot water, etc.), chlorine dioxide generators, solar collectors and gas in the Radom Specialist Hospital complex. VI.36.5 Before awarding the contract, the Contracting Authority shall invite the Economic Operator whose tender was the highest rated to submit the following statements or documents within a specified time limit, which shall not be shorter than 10 days: 6.5.1. List of statements or documents submitted by the Economic Operator in the procedure at the invitation of the Contracting Authority to confirm the circumstances referred to in art. 25 item 1 item 3 of the PPL Act:a) information from the National Criminal Register within the scope defined in art. 24 item 1 items 13, 14 and 21 of the PPL Act, issued not earlier than 6 months before the expiry of the deadline for submission of tenders.Cd. in sec. II.2.4.
The subject matter of the contract are activities ensuring proper functioning in continuous operation of equipment, installations and networks of water, heat (central heating, steam and hot water), central heating system, steam, hot water, chlorine dioxide generators, solar collectors and gas in the hospital facilities: main buildings and structures (A-F segment), SOR, pavilion, gynecology, rehabilitation centre, tuberculosis, infectious diseases, pathomorphology, laboratory, workshop and warehouse, clinic ul. Lekarska, boiler room, oxygen generating plant, Water Treatment Station, sewage sub-treatment plant, Giserska Street rehabilitation, underground tunnels and wells and external networks on the premises of R. Sz. S.Cd. sec. II.1.4:b) declaration of the Economic Operator on the lack of a final court judgment or a final administrative decision on arrears in payment of taxes, charges or social or health insurance contributions or - in the case of such a judgment or decision - documents confirming payment of these charges together with possible interest or fines or conclusion of a binding agreement on payment of these charges;d) an extract from the relevant register or from the Central Register of Business Activity and Information, if separate regulations require entry in the register or register, to confirm that there are no grounds for exclusion under art. e) if the Economic Operator has its seat or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in:1) letter a) - the Economic Operator shall submit information from a relevant register or, if there is no such register, another equivalent document issued by the competent judicial or administrative body of the country where the Economic Operator has its seat or place of residence or the person to whom the information or document refers, within the scope defined in art. 24 item 1 points 13, 14 and 21 of the PPL Act,2) letter d - shall submit a document or documents issued in the country where the Economic Operator has its registered office or place of residence confirming that no liquidation or bankruptcy has been opened.3) The documents referred to in points 1) and 2) should be issued not earlier than 6 months before the deadline for submission of tenders.4) If in the country where the Economic Operator has its seat or place of residence or a person to whom the document refers is not issued, the documents referred to in item 1 shall be replaced by a document containing, respectively, the Economic Operator's declaration, indicating the person or persons authorized to represent him, or a declaration of a person to whom the document was to refer, made before a notary public or before a judicial, administrative or professional or business self-government body competent for the Economic Operator's seat or place of residence or a place of residence of such a person.5) The Economic Operator having a seat within the territory of the Republic of Poland, with reference to a person having a place of residence outside the territory of the Republic of Poland, to whom the document indicated in item a relates, shall submit the document referred to in item 1 within the scope defined in art. 24 item 1 points 14 and 21 of the PPL Act. If in the country in which the person to whom the document was to apply is residing, such documents are not issued, they are replaced by a document containing a statement of that person made before a notary public or before a judicial, administrative or professional or economic self-government body competent for the place of residence of that person.Cd. in sec.
