The subject matter of the contract is the development and production of educational films in the scope of the training activities carried out under the project entitled: "Rational decisions in the health care system, with particular emphasis on the regional health policy", co-financed by the European Social Fund from the Operational Programme Knowledge Education Development 2014-2020: 5th priority axis "Support for the health area", measure 5.2 "Pro-quality activities and organizational solutions in the health care system facilitating access to affordable, sustainable and high quality health services".
The subject matter of the contract is the development and production of min 125 min educational films in the scope of implemented training activities under the project entitled: "Rational decisions in the health care system, with particular emphasis on the regional health policy", co-financed by the European Social Fund from the Operational Programme Knowledge Education Development 2014-2020: 5th priority axis "Support for the health area", Measure 5.2 "Pro-quality actions and organizational solutions in the health care system facilitating access to affordable, sustainable and high quality health services". Presentation of research methods and analytical tools used by AOTMiT employees in the context of the Minister of Health's orders carried out by AOTMiT; - Presentation of AOTMiT activity against the background of the whole healthcare system in Poland; - Promotion of scientific approach to healthcare system problems: Basket of guaranteed services; Medical devices; Tarification process; Cost accounting standard; Drug prices in theory and practice. Levels of payment; Economic analysis in drug evaluation; HTA (Health Technology Assessment); Budget impact analysis and risk-sharing instruments; Reliable sources of knowledge; QALY (quality-adjusted life year); PPZ (Health Policy Programs); PICO scheme.The execution of the subject matter of the contract should be carried out in accordance with the schedule prepared by the Contractor and approved by the Ordering Party, as well as with creative concepts, scripts and screenplays, in 2D or 3D technology, with the possibility of using actors, in colour version: editing, graphic design, soundtrack, translation into English, adjustment to the needs of the disabled. Details of the order are specified in attachment No. 3 to SWZ - Description of the subject of the order (OPZ), and in attachment No. 6 to SWZ - Specimen agreement. The place of delivery of the films will be the headquarters of the ordering party.Term of payment: 30 days from the date of delivery to the Ordering Party's registered office of a correctly issued VAT invoice each time (after a given reception).The Ordering Party informs that prior to the commencement of the procedure it has conducted preliminary market consultations.Information and documentation dz consultations can be found at:https://bipold.aotm.gov.pl/index.php/zamowienia-publiczne/wtepne-konsultacje-rynkoweKLAUZULA SOCIAL1. The Contractor has appointed a person(s) (minimum one person) to work on the preparation of the Schedule, creative concept and coordination of all activities. The Contractor may change the designated person(s) by submitting a relevant application to the Contracting Authority. The Contracting Authority reserves the right to submit a request to the National Labour Inspectorate to inspect the above mentioned employees for compliance with labour law.2.The Contracting Authority requires from the Contractor that within the performance of the subject matter of the contract the activities of preparation of the Schedule, creative concept and coordination will be performed by an employee/s employed under a labour contract within the meaning of the provisions of art. 22 §1 of the Labour Code (i.e. Dz. 1320), which reads as follows: "By establishing an employment relationship, an employee undertakes to perform work of a specified type for the employer and under its direction, and in a place and at a time designated by the employer, and the employer - to employ the employee for remuneration".The Contractor shall declare in the offer form that it employs or will employ the person/s referred to. Failure to submit the above statement by the Contractor or Subcontractor will result in rejection of the tender on the basis of provisions of the Public Procurement Law, as inconsistent with the content of the SWZ.
