REQUEST FOR QUOTATION _____________________________________ RFQ No. 2018-11-003 PR. No. 2018-09-949 The National Bureau of Investigation (NBI), through its Bids and Awards Committee (BAC), will undertake a Small Value Procurement for the “Preventive Maintenance of the Centralized Air-conditioning System at the NBI Clearance Building (for Calendar Year 2018)” in accordance with Section 53.9 of the Implementing Rules and Regulations of Republic Act No. 9184. SCOPE OF WORK: An on duty operator to monitor the unit during operation. I. WATER COOLED LIQUID CHILLER (2 UNITS) Hourly: a. Hourly Monitoring and reading of voltage, amperage, temperature, pressure and oil level readings at chillers in operation or in standby; Daily: a. Daily operation and routine monitoring on chiller control panel in normal, standby mode with oil at normal level, temperature and system refrigerant prior to start-up; b. Daily operation and routine monitoring on flow switches, temperature sender, transducers and control devices prior to start-up; c. Daily operation and routine inspection of chiller room, cooling towers, chilled and condenser pumps, air handling units, exhaust and fresh air fan and machine rooms; d. Daily maintenance such and cleaning of chillers and machine rooms; Monthly: a. Monthly cleaning of control panels, re-tightening of electrical terminals, inspection and preventive servicing of contractors, breakers Semi-Annually: a. Semi-annual inspections and preventive servicing of contactors, breakers, terminals, pressure, temperature, gauges, refrigerant and oil b. Preventive servicing and descaling of cooler tubes, Y strainer, flow switched and sensors, pressure and temperature gauges Annually: a. Annual preventive inspection of lubrication and refrigerant for moisture contamination and test and replacement if required b. Annual preventive replacement of refrigerant and filters II. INDUCE DRAFT COOLING TOWER 1&2 Daily: a. Daily routine operation and maintenance inspection of motors, sprinklers, fan blade, panel, support to spot/detect any abnormalities. b. Weekly maintenance servicing of cooling tower area. Monthly: a. Monthly preventive maintenance, blow down and servicing of cooling tower basin when required b. Monthly preventive maintenance of fan blade motor and de-clogging of sprinklers c. Monthly preventive inspection and servicing of motor control, re-tighten and check voltage and amperage Semi-Annually: a. Semi-annual preventive maintenance, descaling of film filling, blow down and servicing of cooling tower basin when required. b. Preventive maintenance, blow down of system, servicing of basin, sprinklers, motor, fan blades, fillings support and panelling c. Preventive servicing on valves, pipes strainers and repainting of piping supports and panelling. d. Preventive servicing on chemical tank, dosing pump, feeder lines and chemicals. III. CHILLED AND CONDENSER TUBES AND WATER PUMPS Daily: a. Daily operation and routine inspection on circulating water pumps control panel, voltage, amperage, shaft seal, flow switch, pressure and temperature gauges. Monthly: a. Monthly preventive servicing on circulating water pumps, control panels, check voltage, amperage, shaft seal, pressure and temperature gauges. Perform corrective and preventive repair. Semi-Annually: b. Semi-annual servicing of circulating water pumps, control panels, check voltage and amperage, shaft seal, pressure and temperature gauge, gale values, check value, and strainers, pipe hinges, supports and insulation and perform corrective and preventive repair c. Blow down of close water circulating system when parameter is exceeded and dose shock treatment procedure d. Servicing of an electrical pump controls, gate valves, check valves, steamers, pressure and treatment gauges, piping and insulations, hangers and supports e. Repainting of exposed piping, hangers, pumps and supports b. Annual Repainting IV. AIR HANDLING UNITS (4 UNITS) Daily: a. Daily operation routine inspection on motor, control and blower part prior to start-up. b. Daily operation inspection on water circulating pressure and temperature on the system. c. Daily operation inspection on supply air ducts, diffuser and registers d. Daily operation inspection on control panel for fresh air and exhaust fan Weekly: a. Weekly inspection for preventive maintenance of machine rooms, air blowers, motor, v-belt bearing, filters, temperature and pressure gauges, fresh air fan, piping, valves, coils, drain, supply at return duct. Initiate preventive repair when required. Monthly: a. Monthly preventive maintenance and servicing on blowers, panels, motor controls, belt tensions, machine rooms, return ducts and fresh air fan. Initiate repair when required. Semi-Annually: a. Semi-annual preventive maintenance servicing on valves, strainers, coils, fins, temperature and pressure gauges. Blower and lubrication of bearings, air filters. a. Preventive maintenance servicing. Blow down of water on fins, valves, strainer, coils, drains, belts, bearing, motor, control panels, piping and insulation, fresh air fan, machine room. b. Repainting of spotted corrosion on panels, bracket and supports c. Duct supply and return ducting, grills and diffusers and exhaust fans. Approved Budget for the Contract (Inclusive of applicable taxes) : P 900,000.00 (Nine Hundred Thousand Pesos only) Delivery Location : NBI, Taft Avenue, Manila Delivery Date : 30 Days from receipt of Purchase Order TERMS AND CONDITIONS: A. SUBMISSION OF REQUIREMENTS The interested SUPPLIER shall submit three (3) sets of the following financial, technical and eligibility requirements. 1. FINANCIAL AND TECHNICAL REQUIREMENT i. Interested Suppliers shall accomplish, provide correct and accurate information and submit their duly signed PRICE QUOTATION FORM (See Annex “A”) using their official letterhead. The Price Quotation Form shall be signed by the Owner/Proprietor/President. In the case of representatives, the Power of Attorney or Secretary’s Certificate, as the case may be, shall be attached. 2. ELIGIBILITY REQUIREMENTS i. Supplier shall also submit certified true copies of the following documents: (a) Mayor’s Permit/ Business Permit (b) PHILGEPS Certificate of Registration (c) Latest Income/Business Tax Returns (d) Omnibus Sworn Statement (See Annex “C”) B. PROPER SEALING AND MARKING OF QUOTATIONS Quotations shall be enclosed in a sealed envelope, properly marked as to include (1) the name and address of the National Bureau of Investigation-Bids and Awards Committee (NBI-RBAC), (2) the title and ABC of the project, (3) RFQ Number of the project, and (4) name and business address of the prospective Supplier. The improper sealing and/or marking of the quotations shall not disqualify a prospective Supplier. However, the Bids and Awards Committee shall assume no responsibility for the misplacement of the contents of the improperly sealed or marked quotation. C. PLACE AND DATE OF SUBMISSION OF QUOTATIONS Quotations shall be submitted to the Office of the Regular BAC Secretariat, PPLD Building, NBI, Taft Avenue, Manila not later than 9:30 AM of November 29, 2018. D. OPENING OF QUOTATIONS Quotations shall be opened on November 29, 2018, at 10:00AM, at the 2nd Floor, Conference Room, NBI Main Building, Taft Avenue, Manila. E. AMOUNT OF QUOTATION Price quotations exceeding the ABC shall be rejected. F. VALIDITY OF QUOTATION Price quotations must be valid for a period of thirty (30) calendar days from the date of submission of quotation. G. ALTERATIONS, ERASURES AND OVERWRITING Any alterations, erasures or overwriting shall be valid only if signed or initialed by the bidder or his/her duly authorized representatives. G. NO MULTIPLE SUBMISSION OF QUOTATIONS Multiple submission of quotations shall not be allowed. If during the opening of quotations, a Supplier is found to have submitted two or more quotations for the same project, all submitted quotations of the prospective Supplier shall be disregarded and shall be returned to the latter unopened. H. RIGHT OF INSPECTION The NBI shall have the right to inspect and/or test the goods to confirm their conformity with the minimum technical specifications and other terms and conditions specified herein. I. AWARD OF CONTRACT Award of Contract shall be made to the lowest quotation, which is found to be responsive and compliant with the minimum specifications and other terms and conditions specified herein. J. LIQUIDATED DAMAGES Liquidated damages for late delivery of service equivalent to one tenth (1/10) of one (1) percent of the cost of the unperformed portion of the contract shall be imposed for every day of delay. The NBI shall have the right to rescind the contract when the cumulative amount of liquidated damages reaches ten percent (10%) of the contract price, without prejudice to other courses of action and remedies available to it. K. RESERVATION CLAUSE The NBI reserves the right to accept or reject any or all quotations and to impose additional terms and conditions as it may deem proper.
