Description

The subject of the order is the supply of medicinal products, medical devices, foodstuffs for special nutritional purposes, cosmetics, specified in the sections for the Fryderyk Chopin Clinical Regional Hospital No. 1 in Rzeszów.2A detailed description of the subject matter of the contract, the Awarding Entity's requirements, information on the admission of other quantities of units in packaging is specified in the Price and Tender Forms - Annex 1 for individual parts, which are an integral part of the ToR.3 The Awarding Entity requires that the objects of the contract which meet the requirements described in the ToR.4 The scope of the contract includes the delivery of the objects of the contract in quantities specified in the Price and Tender Forms for the period: The Contracting Authority does not allow submitting equivalent bids in Parts no. 1 to 7, 9 to 11, 14 to 28, 30 to 40, and 44, and allows in Parts no. 8, 12, 13, 29, 41, 42, 43.

In item II.2.7) due to the program are given: 18 months, it should be: Planned delivery of the objects of the contract: for 18 months, counting from 17.12.2020.The Contracting Authority allows for the possibility of earlier purchase in case of exhaustion of the quantity from the current contract. In such a case the contract completion date shall be counted from the moment of the first purchase of a given subject matter of the contract. The Contracting Authority allows for the possibility of a later commencement of contract completion if the tender procedure is prolonged. The Contracting Authority allows for the possibility of changing the contract duration in the case of failure to complete the entire subject matter of the contract within the contract period due to completion of lower orders - the change of the contract requires an annex, however, the Contracting Authority shall unilaterally notify the Contractor about the proposed extension of the contract before the expiry of the term for which it was concluded.
