Description
The subject matter of this public procurement procedure is: "Supply of medicinal products. The subject of the contract was divided into 3 tasks: Task 1 - Miscellaneous products Task 2 - Immonoglobulinum Humanum ANTY-D Task 3 - Miscellaneous medicinal products A detailed description and quantities are presented in Annex 2 of the ToR. Medicinal products must have a validity period of at least 9 months from the date of their delivery to the Ordering Party's registered office. 3. the Awarding Entity allows for partial bids. A partial tender shall be considered to cover one complete task. The Economic Operator may submit a tender for one or for any number of parts of the contract. Each Economic Operator may submit only one tender. 5. a minimum payment term of 60 days is required. Medicinal products which are the subject of an order should be understood as medicinal products within the meaning of the Pharmaceutical Law Act of 6.9.2001 (consolidated text: Journal of Laws of 2008, No. 45, item 271, as amended). The offered medicinal products must be admitted to trading in accordance with the rules set out in Article 3 or 4 or 4a of the Pharmaceutical Law Act. If the medicine being the subject of the order at the moment of valuation is unavailable on the market, the Contractor shall be obliged to value this medicine at the last valid price. 8. medical devices being the subject of the contract should be understood as medical devices within the meaning of the Act of 20.5.2010 on medical devices. The offered medical devices must be approved for marketing and use in accordance with the rules specified in the Act on Medical Devices. (9) Tenders must specify the trade name of the preparation, the name of the manufacturer and the size of the packaging. Deadline for partial deliveries - no longer than 48 hours from placing a partial order by the Pharmacy, on weekdays from 7:30 to 15:00. Delivery time cito is up to 12 hours counting from the date of placing a partial order by the Hospital Pharmacy. If the delivery falls on a non-working day, the delivery shall take place on the first working day after the set deadline. Deliveries made with own transport or rented by the Contractor at his own expense and risk. If the offered medicine packaging contains a different number of units (tablets, ampoules, etc.) than specified in the ToR, the Contractor is entitled to recalculate the number of packages. If, after the calculation, an incomplete number of packages is obtained, the number of packages offered should be rounded up. During the term of the contract, in the absence of the offered preparation on the market, the Contractor is obliged to replace it with a synonymous drug at a price not higher than the tender price (after consultation with the Pharmacy Manager). If the packaging of a synonymous medicine contains a different number of units (e.g. tablets, ampoules, etc.), its price should be recalculated so that the price of 1 unit (tablets, ampoules, etc.) in the packaging is not higher than the price of 1 unit according to the tender contract. (15) A medicine containing the same active substance, at the same dose, with the same indications for use and the same pharmaceutical form shall be considered to be a synonymous preparation. If a description of the medicine was included in the ToR, the synonymous preparation must also correspond to this description. If it is not possible to deliver a synonymous preparation within 48 hours, the Contractor is obliged to issue a written note, which will be the basis for the purchase of the medicine in another pharmaceutical wholesaler and to cover the difference in purchase costs. In the event of a change in the size of the packaging during the term of the contract, the Contractor shall deliver the medicine in a new packaging. The price of the medicine per unit may not be higher than the tender price. The Contractor is obliged to correctly calculate the number of commercial packaging of individual lakes in order to obtain the quantity of tablets/ ampoules/vials/vials/vials/ flacons/ pieces/tubes required by the Ordering Party. a) Capsules for soft capsules b) Capsules for hard capsules c) Capsules for tablets d) Capsules for tablets d) Capsules for coating tablets. The Contractor is obliged to indicate and mark all changes and conversions made in the price form.
