Package 1 - Sekukinumab. Detailed description of the subject of the order is presented in Appendix No. 5 to the ToR - Assortment and price form. Detailed requirements concerning the order: 1) the period of validity of products must be at least 75% of the total length of validity specified by the manufacturer. Deliveries of products with a shorter term of validity may be allowed in exceptional situations and each time the consent to them must be given by an authorised representative of the Ordering Party.2) time of realization of ordinary orders - scoring in accordance with the provisions of the ToR, part XXV3) maximum time of realization of orders "for cito" 24 hours or less, regardless of the value of the order placed, except for Package no. 14 for which the order "for cito" is 8 hours from the order placement4) The Ordering Party allows using an equivalent medicine, that is a replacement medicine (generic)5). The Contractor is obliged to attach the CHPL card to the offer only for medicinal products other than those specified in the assortment and price form attached to the Application. All other CHPL cards not mentioned above shall be provided by the Contractor at the request of the Ordering Party. An equivalent medicine must meet the following conditions:- will have the same dosage and composition in the international name as the original medicine - will have the same active substance as the original medicine - will have the same form of medicine as the original medicine - will have the same pharmaceutical availability as the original medicine - will have the same bioavailability as the original medicine - will have the same bioequivalence as the original medicine - the same pharmacological effect as the original medicine - the same authorisation(s) for use as the original medicine - if an equivalent medicine is offered, that is, if an equivalent medicine is offered, i.e. a medicine with the same bioavailability as the original medicine. The Economic Operator is obliged to offer medicinal products covered by reimbursement by the National Health Fund (the products must be registered in drug programmes) and the Economic Operators must attach to the offer documents confirming possession of all the features indicated in the description of the subject of the contract, i.e. the CHPL (Characteristics of the medicinal product) card for a given equivalent medicine. The Contractor is obliged to attach the CHPL card to the offer only for medicinal products other than those specified in the assortment and price form attached to the Application. All other CHPL cards not listed above shall be provided by the Contractor at the request of the Ordering Party. Subject conditions required from Contractors as described in the ToR, Chapter III.3
