1. the subject of the contract is the successive supply of medicinal products (drug program) to the Hospital Pharmacy of the Employer, Specialist Hospital for Lung Diseases "Rebirth" in Zakopane. A detailed description of the subject matter of the order is contained in Appendix No. 1 to the SWZ (price calculation of the offer).2. The subject matter of the supply must be authorized for marketing and use in the territory of the Republic of Poland in accordance with applicable regulations , in particular: the Act of September 6, 2001. Pharmaceutical Law (i.e. Journal of Laws of 2021, item 1977 as amended).3. The subject of the order is to be delivered in the manufacturer's packaging. 4.The external and internal packaging must contain a minimum of the name of the goods, batch number, and expiration date (if the validity of the product is not determined by a specific date, the date of manufacture is also required).5.The Contractor must guarantee a sufficiently long period of validity of the supplied products ( min. 6 months).
