The subject matter of the contract is the supply of electricity for the needs of the Municipality of Gora, its organizational units, cultural institutions and the company.Statements and documents to be submitted by the Contractor with the bid (completed bid form, the specimen of which is attached to the SWZ) 7.2 The completed JEDZ . A statement in the form of the Single European Procurement Document (hereinafter: JEDZ), current as of the date of submission, completed in the scope indicated by the Contracting Authority in section 6.1 and 6.2 of the SWZ (grounds for exclusion) In accordance with the content of Art. 63 par. 1, the bid and JEDZ shall be submitted, under pain of invalidity, in electronic form 7.2.2 statement of no grounds for exclusion in the circumstances indicated in Art. 7 par. 1 of the Law on Special Solutions for Counteracting Support for Aggression against Ukraine and for the Protection of National Security, and in The statement shall be submitted in electronic form In the case of submitting an electronic document in a compressed data format in a procedure or competition, affixing a qualified electronic signature to a file containing compressed documents is equivalent to affixing a qualified electronic signature to all documents contained in the file, respectively. Article 125(4) In the case of joint bidding, JEDZs shall be submitted by each of the contractors. They shall confirm the absence of grounds for exclusion; 7.3 A power of attorney to sign the contractor's bid, if a person whose power of attorney does not appear from an excerpt or information from the National Court Register or CEIDG acts on behalf of the contractor. A copy or information from the National Court Register or from the Central Register and Information on Business Activity or other appropriate register to confirm that the person acting on behalf of the contractor is authorized to represent him This requirement applies to the Contractor, contractors acting jointly, if the Contractor has not adequately indicated in the statement of Article 125 (1) of the PPL Act the address of the free and publicly available database where they are available and the Contracting Authority may download them independently
The subject matter of the contract includes the supply of energy to approximately 220 PPEE. Projected energy consumption of approximately 1889813 kWh. Subject matter evidence that the Contracting Authority will call upon the Contractor to submit after the deadline for submission of tenders under the rules described in Article 126 (1) of the PPL Act: 1. a copy or information from the National Court Register or from the Central Register and Information on Business Activity, within the scope of Article 109 (1) (4) of the Act, prepared no earlier than 3 months prior to its submission if separate regulations require entry in the register or registry, if the Contractor did not adequately indicate in the statement from Article 125 (1) of the Pzp. 1 of the PPL Act the address of the free and publicly available database where they are available and the Contracting Authority may download them independently/ 2. Information from the National Criminal Register in the scope specified in Article 108 (1) (1), (2) and (4) of the PPL Act, issued no earlier than 6 months before its submission. If the Economic Operator has a registered office or place of residence outside the territory of the Republic of Poland, instead of the above mentioned documents he shall submit: 1) information from a relevant register such as a court register or, if there is no such register, another equivalent document issued by a competent judicial or administrative authority of the country in which the Economic Operator has its registered office or place of residence. 2) The documents referred to in subsection 1) should be issued no earlier than 6 months before their submission. 3) If the documents referred to in subsection 1) are not issued in the country in which the Economic Operator has its seat or residence, or if these documents do not refer to all cases referred to in art. 108 par. 1 (1), (2) and (4) of the PPL Act, they shall be replaced in whole or in part, respectively, by a document containing a statement of the Economic Operator, indicating the person or persons authorized to represent him, or a statement of the person to whom the document relates, made under oath, or, if in the country where the Economic Operator has its registered seat or place of residence there are no provisions on declaration under oath, made before a judicial or administrative body, notary, professional or economic self-government body competent for the seat or place of residence of the Economic Operator. The provision of subsection 2) shall apply accordingly. 3.Statement of the Economic Operator, within the scope of Article 108 (1) (5) of the PPL Act, that he is not a member of the same capital group, within the meaning of the Act on Competition and Consumer Protection of February 16, 2007 (i.e. Journal of Laws of 2021, item 275), with another Economic Operator who submitted a separate bid, or a statement that he is a member of the same capital group, together with documents or information confirming preparation of a bid independently of another Economic Operator belonging to the same capital group. 4.The Contractor's statement on the timeliness of the information contained in the statement referred to in Article 125 paragraph 1 of the Act, with regard to the grounds for exclusion from the procedure indicated by the Contracting Authority, referred to in Article 108 paragraph 1 points 3, 4, 5 and 6, and the timeliness of the statement with regard to the grounds for exclusion under Article 7 paragraph 1 of the Act on special solutions to prevent the support of aggression to Ukraine and to protect national security and under Regulation 2022/576 5.Documents prepared in a foreign language must be submitted with a translation into Polish.
