Description

The subject matter of the order is the delivery of orthopaedic prostheses and surgical implants within 48 hours from the time of placing the order as well as providing instruments for SP ZOZ in Łęczna to the Operating Theatre.3.2 A bank of implants and prostheses constituting the subject matter of the order shall be created at the Ordering Party's registered office and it shall be owned by the Contractor and it shall be successively supplemented by the Contractor.3.3 A detailed description of the subject matter of the order is presented in the price form constituting Appendix 2 to SWZ (respectively). 3.1 The subject matter of the order is the delivery of orthopaedic prostheses and surgical implants within 48 hours of the order being placed and the provision of instrumentation for SP ZOZ w Łęcznej to the Operating Theatre.3.2 A bank of implants and prostheses constituting the subject matter of the order will be created at the Ordering Party's registered office; it will be the property of the Contractor and it will be subject to successive replenishment by the Contractor.3.3 A detailed description of the subject matter of the order is given in the price form constituting attachment no. 2 to SWZ respectively. Within a set all listed items should be offered. Failure to fill in any item within a part shall result in rejection of the offer.3.8. The subject matter of the order shall be new, admitted to circulation pursuant to applicable law and meet all requirements defined by law, in particular the Act of 20 May 2010 on medical devices (Journal of Laws of 2021, item 1565) and be free from any physical or legal defects.3.9. Whenever trade marks, patents, origin, standards, approvals, technical specifications and technical reference systems are indicated in the description of the subject matter of the contract, equivalent solutions shall be allowed by the Contracting Authority. 3.11. The Ordering Party shall allow for offering the subject matter of the contract in packages containing quantities of products other than those suggested in tables of price forms, with appropriate conversion of quantities, rounded up to 2 decimal places.3.12. The payment term shall be up to 60 days from the date of receiving a correctly issued invoice.3.13. Information about prior evaluation of the offers, pursuant to art. 139 of the PPL. The selection of the most advantageous offer will be made using the so-called "reverse procedure". The Contracting Authority shall first examine and evaluate the tenders and then qualify the contractors whose tenders received the highest scores regarding no grounds for exclusion and meeting the conditions for participation in the proceedings
