Description

The AOK Nordost plans bindingly coordinated and structured cooperation between full inpatient care facilities and primary care physicians who already provide services for the care facility's insureds under a cooperation agreement pursuant to Section 119b of the German Social Code, Book V (SGB V). The aim of the contract is to provide participating insureds in need of care with binding, close-knit, high-quality, needs-based medical and nursing care. To this end, contracts are concluded in accordance with § 140a SGB V in conjunction with § 92b SGB XI with providers of fully inpatient care facilities, which go beyond the services in accordance with § 119b SGB V.This is an approval procedure within the meaning of recital (4) of the EU Directive (2014/24/EC) of 26.02.2014 on public procurement. Any suitable interested party may become a contractual partner. Subject to uniform suitability requirements and taking into account the principle of economic efficiency (§§ 4 (4), 12, 70 SGB V), all suitable full inpatient care facilities are offered the conclusion of a contract in accordance with § 140a SGB V in conjunction with § 92b SGB XI, as long as the objectives of the contract can still be achieved at the time the contract is concluded.
The AOK Nordost publishes the approval procedure of the special care for insured persons in need of care in full inpatient care facilities for the period from 01.04.2022 to 31.03.2026, which are approved according to § 71 para. 2 SGB XI. The care of the participating insured persons in need of care, most of whom are multimorbid, is to represent binding, close-meshed and high-quality, needs-based, medical and nursing care within the framework of this contract.To this end, binding regular attendances of the cooperating family doctor, extended on-call family doctor services as well as weekend meetings for the agreement of individual emergency plans for participating insured persons with foreseeable, emerging acute situations are defined.In order to implement more effective care processes, the nursing service providers (contractually bound full inpatient nursing facilities) carry out a coordinated and binding cooperation with the cooperating family doctor in accordance with this contract. This takes place, among other things, through regular case conferences and doctor-patient contacts accompanied by the nursing facilities, coordinated and binding availability as well as the appointment of a fixed contact person in the full inpatient nursing facility for the cooperating family doctor.For the interfaces of medical and nursing care, a concept is to be developed by the nursing service providers in which the distribution of tasks, the responsibilities as well as the coordination processes are depicted.The nursing service providers must ensure that the nursing staff completes the training offered by the payer within the framework of this contract.The training includes recognizing the drug-associated illnesses and risks on the basis of symptoms of insureds in need of care and reducing and/or avoiding them as far as possible, as well as creating the basis for targeted communication with all those involved in care.The nursing service providers care for the insureds and are responsible for involving the cooperating primary care physicians (in accordance with point III.1..3 d).The nursing and medical service providers have to fulfill personal and technical prerequisites as well as quality requirements, which are described in the application for participation (including forms).Remuneration is based on a flat rate per quarter.The form for the open procedure is used only because there is no form for admission procedures. Contracts that do not involve a selection decision among eligible applicants are not subject to procurement law. Use of this form does not imply submission to procurement law regulations.
