Description
The subject of the contract is: provision of services in the scope of successive collection from the premises of the Customer, transport and disposal of medical waste with codes: 18.01.03, 18.01.02, 19.08.01, 18.01.81, 18.01.82, 18.01.06, 18.01.01, 18.01.01, 18.01.07, 18.01.08, 18.01.09 during the term of the agreement. Waste collection will be carried out five times a week from the following locations: - Regional Hospital in Kielce, main building - Utylizacji Warehouse, Kielce, ul. Grunwaldzka 45, - Regional Hospital in Kielce - Infectious Diseases Clinic, Kielce, ul. Radiowa 7, - Regional Hospital in Kielce - Rehabilitation Clinic, Kielce, ul. Kościuszki 3,- Provincial Hospital in Kielce - Pediatric Hospital, pavilion G, Kielce, ul. Artwińskiego 3a The estimated weight of medical waste to be disposed of during the order completion period is 480,000 kg. Information on the principles of contract performance and remuneration is included in Appendix No. 3 to the ToR - template agreement.
