Description of the project:The project "Optimized acute care of geriatric patients through an intersectoral telemedical cooperation network - around the clock (Optimal@NRW)" establishes an innovative intersectoral approach on the basis of which the cooperation of the outpatient and inpatient sector, the geriatric care and the emergency services can be carried out in a more patient-centered way.This project takes up the changes of the emergency reform currently planned in Germany, which are listed below (as of March 2020):- Introduction of a joint emergency control centre consisting of 112 and 116117, which is to be accessible by telephone 24/7. There, the urgency of the situation will be assessed by means of standardised initial medical assessment (SmeD) and the information will be forwarded to the appropriate level of care (e.g. outpatient CT emergency service, ambulance service, hospital, etc.).- Integrated emergency centres (INZ) are to be set up at selected hospitals. These will be staffed by outpatient and inpatient physicians in the sense of a joint "one-desk" model. The INZs will be a point of contact for patients and will be responsible for further coordination when patients visit them.
The object of the contract is the creation, parameterisation and licence acquisition of an electronic patient file, including the provision of operational readiness and system service. The development of interfaces to various systems is also subject matter of the contract. For the Principal, it is of essential contractual importance that the Contractor produces the functionality of the electronic patient file agreed in this contract and takes all necessary steps for this. "Services up to acceptance":- License acquisition (according to 1.3.),- Interface development and connection to:-- electronic nursing documentation systems (Connext Vivendi, DAN, DM7),-- practice management systems (see catalogue of requirements),-- an early warning system or the associated information management system,-- documentation software used in the context of teleconsultations (TeleDoc software/MedCast Cloud of Docs in Clouds TeleCare GmbH),- creation/production and bringing about operational readiness (e.g. through installation and integration of the system components),- training,- project management (continuous contact person)"Services after acceptance":- system service (e.g. maintenance and/or restoration of operational readiness, user administration),- further development and adaptation of the overall system,- provision of data stored in the patient file for a study database.
