Description

The subject of the contract is: Delivery of medical equipment Task 1 - Esmarch apparatus Task 2 - Surgical diathermy Task 3 - Operating table Task 4 - Defibrillator The Ordering Party allows submitting partial offers. Parts of the contract are the individual tasks specified above, i.e. tasks 1 to 4. The Contracting Authority hereby informs that a Bidder may submit a bid for up to 4 tasks (without limiting the number of tasks per one Bidder).
3.3 The Ordering Party's requirements regarding the subject matter of the contract: 1) The offered medical device must have approvals, certificates and be registered in accordance with the law requiring the possession of approvals, certificates and registration,2) The offered medical device must be marketed and used in accordance with the requirements of the Act of 20 May 2010 on medical devices (Journal of Laws of 2021, If the offered medical device does not require registration with the President of the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products, the Contractor shall be obliged to perform at his own expense and within his own scope all activities necessary to meet all legal requirements related to placing the device on the market and using it in the territory of the Republic of Poland. In the cases where the description of the subject matter of the contract includes trademarks, patents or origin, source or specific process that characterizes certain products or services provided by a particular economic operator, it means that the Contracting Authority cannot describe the subject matter of the contract in a sufficiently precise and comprehensible way, however, in each such situation the Contracting Authority shall accept solutions equivalent to those described above. In such situations any reference to trademarks, patents, origin, source or a specific process that characterizes certain products or services provided by a particular economic operator shall be read together with the words "or equivalent". When describing the subject matter of the contract by reference to standards, technical evaluations, technical specifications and technical reference systems, referred to in art. 101 par. 1 item 2 and par. 3, the Contracting Authority shall allow for solutions equivalent to those described. The draft provisions of the public procurement contract to be introduced into the contract are annexed to the SWZ. 214 PAR. The Contracting Authority does not anticipate awarding any contracts as referred to in Article 214(1)(7) of the PPL Act. DESCRIPTION OF CRITERIA WHICH THE CONTRACTING AUTHORITY WILL TAKE INTO ACCOUNT WHEN SELECTING A TENDER'S OFFER, INCLUDING THE WEIGHT OF THESE CRITERIA AND THE METHOD OF EVALUATING TENDERS13.1.During the evaluation of tenders, each evaluated tender will be awarded points for the following criteria, according to the following rules: PRICE CRITERION weighting: 60% in relation to tasks no. 1, 2, 4 PRICE CRITERION weighting: 50% in relation to task no. 3 PERFORMANCE TIMEFRAME weighting: 20% in relation to all tasks GUARANTEE CRITERION weighting: 20% in relation to all tasks QUALITY CRITERION weighting: 10% - in relation to task no. 3
