The subject of the contract is the delivery of medical equipment and apparatus under the project no. POIS.09.02.00-00-0164/19 entitled: "Onkoinnowacje".2 Body composition analyzer.3The Contractor shall deliver the subject of the order to the Principal's registered office or shall deliver and install it at the Principal's registered office (depending on the type of equipment), using its own means of transport and at its own expense, also bearing the cost of loading and unloading and insurance during transport, shall make the first start-up and, within the time agreed with the Principal, shall train the personnel indicated by the Principal in the scope of operation and maintenance or shall provide instructions, in accordance with the requirements contained in Annex 8 to the ToR.4A detailed description of the subject matter of the contract is provided in Appendix No. 2 to the ToR - Assortment and price form and Appendix No. 8 to the ToR - Technical requirements form - limit conditions.5. The offered subject of the order must be brand new, manufactured after 1 January 2019 and admitted to marketing and use, in the case of medical devices in accordance with the Act of 20 May 2010 on Medical Devices (consolidated text Journal of Laws of 2019, item 175, as amended), have the CE mark to confirm that they meet all relevant legal requirements in force in the EU, including those relating to safety, health protection or the environment.6. The offered subject of the order must be brand new, manufactured after 1 January 2019 and admitted to marketing and use, in the case of medical devices in accordance with the Act of 20 May 2010 on Medical Devices (consolidated text Journal of Laws of 2019, item 175, as amended), have the CE mark to confirm that they meet all relevant legal requirements in force in the EU, including those relating to safety, health protection or the environment.6. With the handover, the Contractor shall provide appropriate documents allowing for use, including for the medical range, the Declaration of Conformity, manufacturer's documentation, instruction manual and guarantee for the delivered object of the order. POIS.09The Contractor shall deliver the subject of the order to the Customer's registered office or shall deliver and install at the Customer's registered office (depending on the type of equipment), using its own means of transport and at its own expense, also bearing the cost of loading and unloading and insurance during transport, make the first start-up and, within the time agreed with the Customer, train the personnel indicated by the Customer in the field of operation and maintenance or provide instructions, in accordance with the requirements contained in Annex 8 to the ToR.4.A detailed description of the subject matter of the contract is provided in Appendix No. 2 to the ToR - Assortment and price form and Appendix No. 8 to the ToR - Technical requirements form - limit conditions.5.
