Description
The subject matter of the contract is the provision of laundry services together with the lease of assortment and implementation and maintenance of the RFID system or equivalent for Sosnowiec Municipal Hospital Ltd. A detailed description of the subject matter of the contract is included in Attachment 1.1 to the SWZ. The assortment and quantities are shown in Attachment 3 to the SWZ. Bids submitted must include the full scope of the subject matter of the order. 3 Requirements for the subject matter of the contract: The Contractor shall perform services consisting of; 3.1 performing a comprehensive service of washing, drying and treatment (ironing, mangling, disinfection, etc.) of hospital linen, surgical linen, pillows, blankets/quilts, mattresses, mattress covers, pillows, quilts/blankets and other such as. The service includes the repair and dry-cleaning of items not suitable for water-based laundering and clothing of patients from the Care and Treatment Centre. 3.2. .... vide attachment 1.1 to SWZ
3.1 performance of comprehensive washing, drying and processing (ironing, mangling, disinfection, etc.) of hospital linen, surgical linen, pillows, blankets / quilts, mattresses, covers for mattresses, pillows, quilts / blankets and other such as 3.1. lease of surgical linen, pillows, blankets / duvets, mattresses, mattress covers, pillows, quilts / blankets and other such as curtains, drapes, cloths, tablecloths, cloths, mops, together with repair and dry-cleaning of assortment not suitable for water laundering and clothes of patients from the Care and Treatment Unit. Lease of hospital linen, including duvet covers, pillowcases, bedspreads, pillowcases, quilts/blankets, covers for pillows and quilts/blankets, barrier linen - in accordance with the assortment and conditions specified in Annex 3 to the SWZ Price and Assortment Form. 3.3. implementation of an RFID system or equivalent allowing for identification and registration of the assortment to be laundered from individual wards, enabling contactless counting..4. permanent marking of the assortment owned by the Purchaser and as indicated by the Purchaser with RFID tags or equivalent with the following data: name of the hospital, name of the assortment e.g. duvet cover, quilt/blanket, department name and others as required.3.5. the Contractor shall be obliged to present to the Purchaser a written concept of practical implementation of the system and performance of services.3. Before concluding the contract and for the duration of the contract, the Contracting Authority shall have the right to verify the information provided in the tender and to inspect the conditions under which the service is performed by visiting the Contractor's laundry8. 4) Manufacturer's leaflets specifying the duration of action and the required concentration of the preparation in the working solution.5) Current certificates of approval for marketing and use, i.e.: a) for products qualified as medical devices - a declaration of conformity with the essential requirements of CE and - if required by separate regulations - a certificate from the notified body that participated in the evaluation of the medical device b) for products qualified as biocides - a permit referred to in the Act on biocides (Dz.U. of 2002 No. 175, item 1433) c) documents confirming the effectiveness of the disinfectants used to carry out the contract, in accordance with EN-PN standards at least Phase 2, Stage 1 against bacteria, fungi, viruses, mycobacteria and bacterial spores (in accordance with EN 17126 standards confirming the killing power at least against Clostridium difficile spores). (d) information from the manufacturer of the detergent that the detergent is approved for use in newborn linen. (6) description of washing procedure and technology, i.e. washing of the newborn and preterm delivery ward linen,- washing of the operating suite linen,- washing of reusable surgical clothing, including barrier linen,- treatment of linen contaminated with spores of bacteria such as Clostridium difficile or multi-drug resistant pathogens, including CPE, KAR-R,- sterilization of linen packages for the neonatal and preterm delivery ward and operating suite.7) a copy of the policy". The Ordering Party requires that the Contractor has civil liability insurance throughout the duration of the contract on the terms specified in ยง 11 of the model contract - Annex 4 to the SWZ (Terms of Reference). A copy of the policy shall be attached by the Economic Operator together with signing the contract.
