Description

The subject of the contract is the delivery of an automated electrophoresis system with the lease of the analyzer for a period of 4 years for the Independent Public Healthcare Facility of the Provincial Specialist Hospital No. 3 in Rybnik. The order specification is contained in Annex 1.1.A-1.1.C. While describing the subject matter of the order, the Ordering Party defined in detail the assortment in accordance with the needs of a hospital, it also allows for an equivalent product, which is understood as an assortment of identical purpose as described in Annex 1.1 ("Price Form") and parameters in accordance with Annex 1.2. not worse from the point of view of the user.The submitted offer must be complete, i.e. it must include the assortment allowing for the execution of all types, in accordance with all requirements of the ordering party. The contractor's obligation (without separate charges, except for the lease price) is to maintain the system in full working order, i.e. full maintenance service - in accordance with the conditions specified in the contract. The quantities stated in the price forms refer to the period of 48 months or until the contract value is exhausted, depending on which event occurs first. These quantities are indicative and may vary depending on the needs of the hospital. Deliveries will be made by transport at the cost of the contractor according to the rules set out in the contract. The rules set out therein indicate, inter alia, obligations regarding the manner and time of delivery.Other terms of the order are set out in Appendix 2 to the Specification of Key Terms of Purchase ("Model Contract").1. the range of products offered must: 1. conform to the Specification of Key Terms of Purchase - including all requirements specified in the price form;2. 3. have a product range expiry date not shorter than 3 months from the date of delivery, unless otherwise stated in the price list (applies to product range for which the expiry date is set).
