Description

The subject matter of the contract is successive supply, according to the Ordering Party's needs, of laboratory reagents for biochemical tests, together with lease of an analyser.The Economic Operator is obliged to submit a statement on a standard JEDZ form, under pain of invalidity, in an electronic form. In the case of joint bidding by Economic Operators, the JEDZ statement shall be submitted by each of them.The Economic Operator who relies on abilities or situation of entities making available resources mentioned in art. 118 of the PPL Act, shall submit a statement in the form of JEDZ of the entity making available the resources, confirming no grounds for exclusion of this entity and meeting the conditions of participation in the proceeding or selection criteria, respectively, in the scope in which the Economic Operator relies on its resources.
Detailed description of the subject matter of the contract has been included in the assortment and price form constituting attachment no. 2 to the SWZ and attachment no. 3 to the SWZ - Specification of technical and application parameters for the analyzer and reagents for biochemical tests, and the terms and conditions of contract performance are specified in the draft agreement constituting attachment no. 5 to the SWZ, 1) Confirmation that the medical devices being the subject matter of the order meet the requirements specified in the act of 20.05.2010 on medical devices 1)Confirmation that the medical devices being the subject matter of the contract offered meet the requirements set forth in the Medical Devices Act of 20.05.2010 - documents confirming the marketing authorization for each product offered in the form of a Declaration of Conformity issued by the manufacturer, CE Certificate issued by the notification body (if applicable) and a Notification / Notification Form to the President of the Office; if the products covered by the contract have not been classified by the manufacturer as medical devices within the meaning of the Medical Devices Act, and therefore are not subject to the provisions of the said Act or European directives, the Purchaser waives the requirement to provide the documents described above.2)catalogue or folder (or extract from the catalogue or folder) confirming that the offered analyzer is consistent with the requirements of the Purchaser, including the name of the manufacturer, product name, catalog number, so that it can be identified. It is recommended that the Economic Operator indicates the individual parameters and functions required by the Purchaser by marking them in the catalogue or folder. If the catalogues (folders) attached to the offer do not present an identical product as the one offered or do not contain all parameters required by the Contracting Authority, this discrepancy should be indicated and it should be demonstrated (described) how the offered analyzer meets the requirements defined in SWZ;3)catalogue or folder (or extract from the catalogue or folder) containing the name of the manufacturer, product name, catalogue number of the offered reagents, calibrators, controls, consumables and accessories.The data concerning the offered parameters, features, realization conditions, contained in the offer and the documents attached to it (e.g. information contained in leaflets, manuals and catalogues) must be identical.Completed and signed by persons authorized to represent the Bidder:1)Offer form, whose specimen constitutes attachment No. 1 to SWZ - filled in and signed by the Bidders,2)Assortment and price form, whose specimen constitutes attachment No. 2 to SWZ - filled in and signed by the Bidders,3)List of technical and utility parameters for the analyzer and reagents for performing biochemical tests, the template of which constitutes attachment No. 3 to the SWZ - completed and signed by the Economic Operator4)Declaration in the form of a Single European Procurement Document (JEDZ) according to the template of the standard form defined in the Commission Implementing Regulation (EU) 2016/7 of 5 January 2016 establishing a standard form for the Single European Procurement Document (Official Journal of the European Union L 3 of 06.01.2016, page 16).6)Third-party commitment (if any).7)Power of attorney to submit (sign) the bid and its appendices on behalf of the Economic Operator - if applicable. The power of attorney shall be submitted in the form of an original bearing the signature of the principal or a notarized copy.
