SUPPLY OF LABORATORY REAGENTS WITH LEASE OF ANALYZERS FOR HEMATOLOGICAL TESTSThe contractor may offer only products that are medical devices within the meaning of the Act of April 7, 2022 on medical devices (i.e., Journal of Laws of 2022, item 974) and intended for in vitro diagnostics.A list of reagents, control materials and consumables is included in Appendix 1 - assortment and price form.A list of required technical parameters of analyzers and reagents is included in Appendix 1a - description of the subject of the order.A list of evaluated parameters of the main analyzer and reagents is included in Appendix 1b - evaluated parameters.
SUPPLY OF LABORATORY REAGENTS WITH LEASE OF ANALYZERS FOR HEMATOLOGICAL TESTSThe subject of the contract includes:(a) lease of analyzers for hematology tests with equipment (main - automatic hematology analyzer type CBC-5DIFF with a technologically and reagent-compatible back-up instrument, constituting a unified validated analytical system), working with the same reagents, except for reticulocyte reagent with accessories, during a period of 24 months (b) successive supply of reagents, control materials and consumables for the offered analyzers during a period of 24 months;c) buyout of participation in the Randox RQ 9118 B external laboratory control program - cycle 58, 59, 60, 61; EQAhem-SOWA-med.: microscopic evaluation of peripheral blood smear 1x per year;d) provision of a starter package to start up the analyzers and provide initial instruction and training of employees designated by the Employer in the proper operation of the leased analyzers;e) connection of the analyzers to the laboratory information system that works with the hospital system (ProfLAb-LIS /Clinninet-HIS);f) maintenance services during the lease period of the analyzers and equipment according to the terms and conditions specified in the SWZ;g) provision of substantive care in the interpretation of test results with personal and contact details of the person responsible in the above-mentioned scope for the entire period of use of the analyzers.
