The controller of your personal data is Samodzielny Publiczny Zakład Opieki Zdrowotnej Ministerstwa Spraw Wewnętrznych i Administracji z Warmińsko-Mazurskim Centrum Onkologii based in Olsztyn at 37 Wojska Polskiego Avenue;2. Your personal data will be processed on the basis of Article 6(1)(c) of the RODO for the purpose related to the proceedings for the award of a public contract marked ZPZ-07/02/21 - collection, transport, disposal of medical waste;4. recipients of your personal data will be persons or entities to whom documentation of the proceedings will be made available on the basis of Article 18 and Article 74 of the Act of 11 September 2019. - Public Procurement Law (Journal of Laws of 2019), hereinafter referred to as the "PPL Act";5. Your personal data will be kept, pursuant to Article 78(1) of the PPL Act, for a period of 4 years after the end of the procurement proceedings;6. The obligation for you to provide personal data concerning you directly is a statutory requirement under the PPL Act, related to participation in the public procurement proceedings; consequences of failure to provide certain data arise under the PPL Act;7. 7. with regard to your personal data no automated decision will be taken pursuant to art. 22 RODO;8. you have the following: - pursuant to art. 15 RODO the right to access personal data concerning you,- pursuant to art. 16 RODO the right to correct your personal data *,- pursuant to art. 18 RODO the right to demand from the controller the restriction of processing of personal data right to lodge a complaint to the President of the Office for Personal Data Protection, if you consider that the processing of personal data concerning you violates the provisions of RODO;9. 9. you are not entitled:- in relation to Article 17 (3) (b, d or e) of the RODO the right to erasure of personal data,- the right to the portability of personal data referred to in Article 20 of the RODO,- pursuant to Article 21 of the RODO the right to object to the processing of personal data, because the legal basis for the processing of your personal data is Article 6 (1) (c) of the RODO.* Clarification: the exercise of the right to rectification shall not have the effect of modifying the result of the public procurement procedure or modifying the provisions of the contract to an extent contrary to the PPL Act and shall not affect the integrity of the protocol and its annexes ** Clarification: the right to restrict processing shall not apply in relation to storage, to ensure the exercise of legal remedies or to protect the rights of another natural or legal person, or on grounds of important grounds of public interest of the European Union or of a Member State.
