Description

The subject matter of the contract is the reconstruction, extension and repair of the Zakład Opiekuńczo-Leczniczego i Rehabilitacji Medycznej at 42 Mogileńska Street in Poznań by an additional wing with built-in equipment in accordance with the description of the subject matter of the contract specified in Appendix 2 to these Terms of Reference. The description of the subject of the contract, constituting attachment no. 2 to the specification, describing the scope of performed works being the subject of the contract, consists of: 1) Detailed design (industry: land development, architecture, construction, electrical, low-current, sanitary, road, interior design, renovation of building B) with attachments and visualizations, 2) Construction design (industry: land development, architecture, construction, electricity, sanitary, road construction, together with the attachments,3) Technical specification for the execution and acceptance of works (industry: architecture, road, electricity, low-current, sanitary),4) Before the completion of works (industry: construction, renovation of building B, road, electricity, low-current, sanitary, sanitary, land development and greenery),5) Decision No. 908/2018 of the 11th of February 2012.The subject of the investment is the reconstruction, extension and repair of the Zakład Opiekuńczo-Lecznicze i Rehabilitacji Medycznej at 42 Mogileńska Street in Poznań by an additional wing with built-in equipment. The investment is located on plot No. 1/20, Main Ark precinct. 33, the City of Poznań, owned by the City of Poznań, and used by the Zakład Opieńczo-Leczniczy i Rehabilitacji Medycznej, Samodzielny Publiczny Zakład Opieki Zdrowotnej (Free right of use, notarial act 10121/2004 of 30.9.2004). The purpose of the investment is to create a modern facility called Zakład Opiekuńczo-Leczniczy i Rehabilitacji Medycznej, Samodzielny Publiczny Zakład Opieki Zdrowotnej, providing comprehensive medical care for chronically ill people and people in need of rehabilitation.For this task it is necessary to divide the investment execution phase into 2 stages: stage 1 - construction of a four-storey, basement building (A1), added to the existing building (A2), construction of a transformer station (D), reconstruction of the technical infrastructure, land development; stage 2 - reconstruction and extension of the existing main building (A2), reconstruction of the technical infrastructure, land development near the building.The subject of the investment is the reconstruction, extension and repair of the Department of Care and Medicine and Medical Rehabilitation at 42 Mogileńska Street in Poznań by additional wing with built-in equipment. The investment is located on the plot No. 1/20, Main Ark precinct. 33, the City of Poznań, owned by the City of Poznań, and used by the Zakład Opieńczo-Leczniczy i Rehabilitacji Medycznej, Samodzielny Publiczny Zakład Opieki Zdrowotnej (Free right of use, notarial act 10121/2004 of 30.9.2004).
