Description of the subject of the contract for part 1 of the contract:1) Preparation of design and cost estimate documentation for the extension of the internal road at the Tax Office in Myszków, with a length of approx. 270 m, with the following assumptions: a) Road category - internal municipal road, b) Traffic category KR-4,c) Axle load - 100 kN,d) Asphalt concrete road surface,e) Roadway width 6.50 m along the entire length or width 6.50 m along the section length of the road - 1.5 m. Approximately 160 m from the provincial road and 6.00 m further down the road. (The designer will submit for approval concepts for both solutions),f) Roadsides 0.50 - 0.75 m each with gravel surface or a safety band made of paving stones,g) The pavement from Pułaskiego Street to the Tax Office remains unchanged (correction of pavement nullification is allowed),h) Drainage of the road with an open, fortified trench or rainwater channel to the drainage trench as indicated in the graphic attachment, i) Parking bay for parallel parking for 10 passenger cars opposite the Tax Office building in Myszków;2) The subject matter of the contract shall consist ofa) preparing complete design and cost estimate documentation and Technical Specifications for the execution and acceptance of construction works, b) obtaining, on behalf of the Contracting Authority, all industry agreements, consents, opinions and administrative decisions, including the decision on the building permit, c) Copyright supervision over the execution of the investment task in question, 3) The subject matter of the contract for Part 1 of the contract has been precisely described in Appendix 1 to the ToR - detailed description of the subject matter of the contract for Part 1 of the contract.The Contracting Authority shall demand that the offer includes the person(s) designated to perform the contract, who have: 1) construction rights to design in road engineering specialisation without limitation or in a limited scope, within the meaning of the Act of 7.7.1994. - Construction Law (i.e. Journal of Laws of 2018, item 1202, as amended) and the Regulation of the Minister of Transport and Construction of 11 September 2014 on independent technical functions in construction (Journal of Laws of 2014, item 1278, as amended) or equivalent rights acquired on the basis of previously binding provisions, if newer provisions have not repealed the validity of these rights, or equivalent rights acquired outside Poland and recognised under the provisions of the Act of 22.12.2015 on the rules of recognition of professional qualifications acquired in the Member States of the European Union (i.e. Journal of Laws of 2018, item 2272), which will perform personally for part 1 of the contract: - construction project of road extension at the Tax Office in Myszków.and demonstration of the experience of these persons in the offer.The required data concerning the persons indicated and their experience is provided by the Economic Operator in the tender form - Annex No. 4 to the ToR. Description of the subject of the contract for part 2 of the contract:1) Design of a water supply system section of approx. 1 145 m;2) The subject matter of the contract shall consist of:a) Preparation of complete design and cost estimate documentation and Technical Specifications for the execution and acceptance of construction works, b) Obtaining, on behalf of the Awarding Entity, all industry agreements, approvals, opinions and administrative decisions, including the decision on the building permit (the closed area, the decision on the building permit is issued by the Province Governor of Silesia), c) Copyright supervision over the execution of the investment task in question, 3) The subject matter of the contract for part 2 of the contract has been precisely described in Appendix 2 to the ToR - a detailed description of the subject matter of the contract for part 2 of the contract.The Contracting Authority demands that the offer includes the person(s) designated to perform the contract, who have: 1) building rights to design in the specialty of installation in the field of networks, installations and thermal, ventilation, gas, water supply and sewage equipment without restrictions, within the meaning of the Act of 7.7.1994. - Construction Law (i.e. Journal of Laws of 2018, item 1202, as amended) and the Regulation of the Minister of Transport and Construction of 11 September 2014 on independent technical functions in construction (Journal of Laws of 2014, item 1278, as amended) or equivalent rights acquired on the basis of previously binding provisions, if newer provisions have not repealed the validity of these rights, or equivalent rights acquired outside Poland and recognised under the provisions of the Act of 22 December 2015. about the rules of recognition of professional qualifications acquired in the Member States of the European Union (i.e. Journal of Laws of 2018, item 2272), which will personally perform, according to the submitted offer for part 2 or 3 of the order: - construction and executive design of water supply system in the area of Pawiej Street, - construction and executive design of water supply system in the area of Projektowana Street, and demonstration of the experience of these persons in the offer. Description of the subject of the contract for part 3 of the contract:1) Design of a water supply system section of approx. 450 m;2) The subject matter of the contract consists of: a) Preparation of complete design and cost estimate documentation and Technical Specifications for the execution and acceptance of construction works, b) Obtaining, on behalf of the Contracting Authority, all industry agreements, consents, opinions and administrative decisions, including the decision on the building permit, c) Copyright supervision over the execution of the investment task in question, 3) The subject matter of the contract for Part 3 of the contract is described in detail in Appendix No. 3 to the ToR - a detailed description of the subject matter of the contract for Part 3 of the contract.The Contracting Authority demands that the offer includes the person(s) designated to perform the contract, who have: 1) building rights to design in the specialty of installation in the field of heating, ventilation, gas, water and sewage networks, installations and equipment without restrictions, within the meaning of the Act of 7.7.1994. - Construction Law (i.e. Journal of Laws of 2018, item 1202, as amended) and the Regulation of the Minister of Transport and Construction of 11 September 2014 on independent technical functions in construction (Journal of Laws of 2014, item 1278, as amended) or equivalent rights acquired on the basis of previously binding provisions, if newer provisions have not repealed the validity of these rights, or equivalent rights acquired outside Poland and recognised under the provisions of the Act of 22.12.2015 on the rules of recognition of professional qualifications acquired in the Member States of the European Union (i.e. Journal of Laws of 2018, item 2272), which will personally perform, respectively to the submitted offer for part 2 or 3 of the order: - construction and executive design of the water supply system in the area of Pawiej Street - construction and executive design of the water supply system in the area of Projektowana Street, and demonstrate in the offer the experience of these persons.
