Description

(1) The subject of the contract is the successive supply of Milk and dairy products in accordance with the requirements of food law to the recipients and to the places of delivery, specified in Annex No. 3 to the contract, with a minimum shelf life as specified in OPZ item. 2.2 Detailed quantitative specification is specified in Annex No. 1A to the SWZ. This annex is also a form of calculation of the proposed bid price.3. Detailed description of the subject of the order in terms of quality requirements, shelf life, labeling and packaging are the documents specified in Annex No. 5 to the SWZ, developed by the Military Research and Implementation Center of the Food Service in Warsaw. These annexes are an integral part of the description of the subject of the order and this SWZ.

For the above-mentioned subject of the order, 4 supply warehouses are in force:- Food Warehouse of the Rzeszów Security Group - 5 Lwowska St. Rzeszów.- Food Warehouse of the Nisko Security Group - 20 Sandomierska St. Nisko.-. Food Warehouse of the Security Group Jaroslaw - 3 Maja 80 Street Jaroslaw.- Food Warehouse of the Security Group Przemyśl - 29 Listopada 1 Street Przemyśl.5. Deliveries of the subject of the order will take place with a frequency - up to 3 times a week for each recipient from Monday to Saturday at hours agreed with the recipient.
