Description

The subject matter of the contract shall include delivery of a computer tomograph to the Contracting Authority's registered office, with technical parameters specified in Annexes 1 to ToR 2.. The subject matter of the contract also includes:a) execution of adaptation works of rooms and technical infrastructure necessary for the installation, commissioning and proper functioning of the computer tomography laboratory located in the ground floor building connecting the main body of the hospital (A) with the body of the specialist clinic (E); b) assembly, installation and commissioning of the device together with the performance of all necessary tests and checks;c) training of the Employer's personnel in their operation and maintenance, on dates agreed with the Employer; d) minimum 60-month warranty period and provision of guaranteed maintenance services during the warranty period; e) training of the Employer's personnel in the Employer's seat in the field of operation.3Together with the CT scanner, the Contractor shall provide: a) guarantee (guarantee card together with the guarantee service rules); b) instructions for use in Polish - 2 copies; c) completed technical passport - 2 copies; d) information on the types and frequencies of inspections required or recommended by the manufacturer - 2 copies; e) workstation health and safety instructions - 2 copies; f) current document confirming that the offered subject of the order is a medical product and is approved for marketing and use in Poland in accordance with the applicable legal regulations.
