Package no. 1 - Execution of adaptation works with a design of permanent shielding, together with delivery of a computed tomograph to the X-ray laboratory of SP ZOZ ZOZ Głuchołazy, at 3 Lompy St. Package no. 2 - purchase of a Bodyplethysmograph
Detailed description of the subject matter of the contract along with technical requirements can be found in appendix no. 2 SWZ (Terms of Reference).Delivery of the subject matter of the contract along with installation and staff training in the use of the equipment - X-ray laboratory and Pulmonology Department with chemotherapy subdivision at 3 Lompy St. in Głuchołazy3.4. The Ordering Party's requirements concerning the subject matter of the contract; - the equipment: should be new (year of production max 2021), not used, not being the subject of presentations or exhibitions, the equipment should not have been used before by another user; - the equipment should be complete and not require the purchase of additional elements or accessories to be put into operation and used as intended; - the equipment should be approved for use in the territory of the Republic of Poland in accordance with applicable law, in particular the provisions of the Act of 20 May 2010 on medical devices (i.e. Journal of Laws of 2021, item 1655) and bear the CE mark The Contractor shall provide the Ordering Party, together with the subject matter of the contract, with all documentation necessary for the proper use of the subject matter of the contract, including but not limited to: warranties, instructions, manufacturer's labels, certificates of approval and safety certificates in the Polish language, and the technical passport. Installation of medical equipment within the scope of package no. 1; - making adaptations to the room; - making a design of permanent shielding in the laboratory and performing all the tests required by law allowing the use of the CT scanner; - installation of the scanner 3.7. It is recommended that the Contractor make a site inspection and familiarize himself with the actual conditions of the order. To this end, the offer should be accompanied by a statement confirming the visit, confirmed by an authorized representative of the Purchaser. Specimen declaration constitutes attachment No. 6 to the SWZ. In case of any doubts, the Economic Operator should ask the Contracting Authority for clarification. 3.8 The Contracting Authority does not provide for communication between the Contracting Authority and contractors otherwise than by means of electronic communication, with the exception of situations described in provisions of art. 65 item 1, art. 66 and art. 69 of the PPL.
