Description

The Department of Children and Families (DCF) is aware that nationally and in Wisconsin, children account for the largest portion of individuals involved in the child welfare system due to maltreatment in their home. Abuse, neglect, or other traumatic experiences impairs the healthy development of a child’s brain during a critical period, resulting in both short-term and lifelong negative physical health, social, emotional, and behavioral consequences. This has been identified through extensive research on the Adverse Childhood Experiences (ACEs).

The purpose of this document is to provide interested parties with information to enable them to prepare and submit a proposal for services for the Connections Count Prevention/Intervention Program (Connections Count) to provide an outreach and resource referral program aimed at vulnerable young families. Connections Count is an initiative targeting vulnerable families with children under the age of 18 that reside in the following high-needs communities or neighborhoods:
• A community/neighborhood within Milwaukee County
• A community/neighborhood outside of Milwaukee County
• A Tribal Entity within the State of Wisconsin

The intent of the Connection Count initiative is two-fold:
1. To improve the connectedness of vulnerable families with young children to ongoing formal services, community-based resources, and natural supports; and,
2. To assist these families in accessing benefits and services that could meet their needs.

By connecting vulnerable families with children to needed formal services, resources, and supports, Connections Count will help children continue to be cared for in their own homes and avoid costly hospitalization, emergency department visits, and potential involvement in the child welfare system. Current research on savings and cost-effectiveness of similar initiatives shows promising results.

DCF intends to use the results of this Request for Grant Proposal (RFP) to award a minimum of one contract in each of the following:
• A community/neighborhood within Milwaukee County
• A community/neighborhood outside of Milwaukee County
• A Tribal Entity within the State of Wisconsin

Non-profit organizations, for-profit organizations, local units of government and tribal governments are eligible to submit a response to this RFP.

Your submitted proposals must clearly identify (in Section 6 Program Summary, on Attachment B Proposer Checklist, and on the Cost Proposal Attachment C) area you are proposing to serve (within Milwaukee County; outside Milwaukee County; tribal program. A Proposer may submit a proposal to serve more than one area; however, each response must be submitted separately for each community within either or both geographic areas; see Section 3.3 Multiple Proposals.
