The subject of the order is to provide comprehensive cleaning services for hospital rooms with a total area of 12 204.42 m2 in facilities of the Multispecialist District Hospital S.A. located in Tarnowskie Góry at 47-51 Pyskowicka Street and 21 Opolska Street.
The subject of the order is to provide comprehensive cleaning services for hospital rooms with a total area of 12 204.42 m2 in facilities of the Multispecialist District Hospital S.A. located in Tarnowskie Góry at 47-51 Pyskowicka Street and 21 Opolska Street. Opole 21.2. List of areas to be cleaned: - Main building with the psychiatric ward building at Pyskowicka Street - 11 586.79 m2- Buildings at Opolska Street 21 - 574.23 m2- Medical waste warehouses - 43.40 m23. The subject of the contract covers its scope:a) specialist cleaning and disinfection of the hospital area and hospital equipment,b) 24-hour cleanliness maintenance in the hospital premises,c) transport of medical waste from the place of origin to the place of storage,d) transport of municipal waste from the place of origin to the place of storage,e) washing, disinfection and bedding of patients,f) assistance in meeting the physiological needs of patients, decontamination of sanitary vessels,g) transport of dirty and clean underwear,h) activities related to the management of hospital underwear and clothing in hospital wards,i) transport of materials from the Operating Theatre to Central Sterilisation and back,j) food transport services,k) transport services of materials and equipment,l) transport services of patient's biological material,m) assistance in lifting, transferring and transporting patients,n) participation in activities related to waste management (including in particular: waste labeling, waste storage operations, decontamination of warehouse space, decontamination of waste transport trolleys).o) taking care of constant access to sanitary means and hygienic materials.4The offer must be secured with a deposit in the amount: The tender must be secured with a deposit of: 40 000.00 PLN (forty thousand PLN 00/100).5. The deposit must be paid in time before the deadline for submission of offers.6. The deposit may be paid in one or more of the following forms: a) money: by bank transfer to the Customer's bank account: ING Bank Śląski 18 1050 1230 1000 0023 5096 9750. The transfer should be described as follows: "Comprehensive cleaning service at WSP S.A. in Tarnowskie Góry II, mark: WSP-DZP-2100-40/2020 - WADIUM "b) bank sureties or sureties of a cooperative savings and loan fund, however, the surety of the fund is always a cash guarantee;c) bank guarantees;d) insurance guarantees;e) sureties granted by entities referred to in art. 6b section 5 point 2 of the Act of 9 November 2000 on the establishment of the Polish Agency for Enterprise Development (t.NOTE: if the Economic Operator shall submit a deposit in the forms referred to in item 6 b-e through the Purchase Platform - the Contracting Authority shall require a document to be submitted in electronic form according to the rules specified in chapter 12 of the ToR - with the reservation that it shall be signed with a qualified electronic signature by the Guarantor, i.e. the issuer of the guarantee/security.7The deposit document shall be submitted in the form as established by the guarantor/guarantor, i.e. the original document.8 The contractor applying for the contract shall not be obliged to attach to the defective warranty document either powers of attorney for the persons who signed it on behalf of the guarantor or registration documents proving that such power of attorney was granted by persons authorised to represent that entity. The Contracting Authority has the right to presume that, pursuant to Article 97 of the Civil Code, persons issuing a defective guarantee at the premises of a bank or insurer on appropriate forms of these institutions, act within the framework of the authority to perform such legal actions.
