Description

This order has been divided into the following 3 parts:Part 1 - Successive delivery of bread and confectionery products to the warehouse in Nowa Dęba;Part 2 - Successive delivery of bread and confectionery products to the warehouse in Kielce;Part 3 - Successive delivery of bread and confectionery products to the warehouse in Sandomierz.Contractors competing jointly for the award (consortium / partners in a Civil Partnership) within the meaning of Art. 58 of the PPL Act:1) shall be jointly and severally liable for failure to perform or improper performance of the contract;2) must appoint a proxy to represent them in the proceedings for award of the contract or to represent them in the proceedings and conclude the contract for this contract.
The subject matter of the contract is the successive delivery of bread and confectionery products to the warehouse of the 33 Military Economic Branch located in the city of Nowa Dęba, 1 Anieli Krzywoń Street.The production of the subject matter of the contract, the manner of its packaging and transport must meet the requirements of the applicable, national and EU, food law. These regulations are specified in the appendix to the SWZ - draft contract. Contractors, as food business operators, operating food or catering establishments, are required to ensure food safety in accordance with the principles of the Hazard Analysis and Critical Control Points System, known as the "HACCP system". (a procedure to ensure food safety by identifying and evaluating hazards from the point of view of the health requirements of food and the risk of hazards occurring during the course of all stages of food production and marketing, this system also aims to identify methods to eliminate or reduce hazards and establish corrective actions). Accordingly, the Contractor bidding for the contract must have the following documents up-to-date (including for the duration of the contract), which he shall show upon request of the Contracting Authority: (a) confirmation of the implemented HACCP system in the form of a HACCP certificate issued by companies accredited and authorized by the Polish Center for Accreditation based in Warsaw or the national accreditation organization for the country, or (b) a certificate of the competent authority of the State Sanitary Inspection or the authority of the Veterinary Inspection on the supervision of the application, implemented HACCP system, confirming compliance with the requirements necessary to ensure hygiene in the process of production or marketing of articles subject to the contract and to ensure the proper quality of these articles.The contract for this public procurement will be effective from the date of its conclusion until December 31, 2024. The parties to the future agreement agree that the execution of the order will begin no earlier than January 1, 2024. and will continue until December 31, 2024 or until exhaustion of the amount constituting the maximum remuneration to which the Contractor is entitled for the execution of the contract, in accordance with the contents of the bid submitted by the Contractor.In the event that the Ordering Party exercises the contract executed under the option right, the Ordering Party shall, no later than November 30, 2024. will submit to the Contractor the first written statement on the subject of the contract awarded under the right of option, together with a definition of its scope.The delivery of bread, which is the subject of this proceeding, will take place 6 times a week (from Monday to Saturday), while the delivery of confectionery products will take place according to the needs of the Employer a maximum of 6 times a week.The Bid, declarations and other documents relating to this procedure, including subjective evidence, shall be submitted by the Contractor respectively as an electronic document or a digital representation of the document bearing a qualified electronic signature (a document in data formats listed in the Regulation of the Council of Ministers of April 12, 2012 on the National Interoperability Framework, minimum requirements for public registries and exchange of information in electronic form and minimum requirements for ICT systems - bearing a qualified electronic signature). The bid consists of:1) Bid Form - completed and signed with qualified electronic signature by persons authorized to represent the Economic Operator, drawn up in accordance with the form attached to the SWZ,2) Detailed pricing form (according to the part for which the Economic Operator is applying) - completed and signed with qualified electronic signature by persons authorized to represent the Economic Operator, drawn up in accordance with the form attached to the SWZ. The subject of the order is the successive supply of bread and confectionery products to the warehouse of the 33 Military Economic Branch located in the city of Kielce, 300 Wojska Polskiego Street.Production of the subject of the order, the manner of its packaging and transport must meet the requirements of the applicable, national and EU, food law regulations. These regulations are specified in the appendix to the SWZ - draft contract.
