The subject of the contract is: Supply of disinfectants and dressing materialsSubtask 1 - DisinfectionSubtask 2 - Chlorine dioxide-based concentrate for surface disinfection and medical devicesSubtask 3 - Liquid concentrate for cleaning and disinfecting surfaces and medical devicesSubtask 4 - Alkaline degreasing agent for ovens and cookersSubtask 5 - Washing and disinfecting hands of medical personnel and patients' skin, to be used in medical wardsSubtask 6 - Surface disinfectant - Preparation for surface disinfection Zad.7 - Preparation for surface disinfection and washing Zad.8 - Dry wipes in rolls Zad.9 - Plasters and other means for operating theatresThe Orderer allows submitting partial offers.Parts of the contract are the individual tasks specified above, i.e. tasks from 1 to 9.The Orderer informs that the Contractor may submit an offer for a maximum of 9 tasks. A detailed description of the subject matter of the contract is provided in Appendix 2b to the SWZ (Terms of Reference).
The Ordering Party's requirements regarding the subject matter of the contract (if applicable):1) the medical product offered must have attestations, certificates and be registered in accordance with the provisions of law requiring attestations, certificates and registration,2) the medical product offered must be placed on the market and used in accordance with the requirements of the Act of 20 May 2010 on medical devices (Journal of Laws of 2020, If the offered medical device does not require an application to the President of the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products, the Contractor shall be obliged to perform at his own expense and within his own scope all activities necessary to meet all legal requirements related to placing the device on the market and using it in the territory of the Republic of Poland. In the cases where the description of the subject matter of the contract includes trademarks, patents or origin, source or specific process that characterizes certain products or services provided by a particular economic operator, it means that the Contracting Authority cannot describe the subject matter of the contract in a sufficiently precise and comprehensible way, however, in each such situation the Contracting Authority shall accept solutions equivalent to those described above. In such situations any reference to trademarks, patents, origin, source or a specific process that characterizes specific products or services provided by a particular economic operator should be read together with the words "or equivalent".When describing the subject matter of a contract by reference to standards, technical assessments, technical specifications and technical reference systems referred to in Article 101(1)(2) and (3), the Contracting Authority shall accept solutions equivalent to those described. INFORMATION ON CONTRACTS ENVISAGED, AS REFERRED TO IN ARTICLE 214(1)(7) AND (8) INFORMATION ON CONTRACTS ENVISAGED UNDER ARTICLE 214(1)(7) AND (8), IF THE CONTRACTING AUTHORITY INTEND TO AWARD THESE CONTRACTSThe Contracting Authority does not anticipate awarding any contracts as referred to in Article 214(1)(7) of the PPL Act. DESCRIPTION OF CRITERIA WHICH THE CONTRACTING AUTHORITY WILL TAKE INTO ACCOUNT WHEN SELECTING A TENDER'S OFFER, INCLUDING THE WEIGHT OF THESE CRITERIA AND THE METHOD OF EVALUATING TENDERS13.1. Tenders will be evaluated on the basis of the criteria defined in the ToR and their weightings. Offers will be evaluated on the basis of points. During the evaluation of the offers the following criteria will be awarded points for each of the considered and evaluated offers, according to the following rules:For all Tasks PRICE CRITERION weighting: 60% PARTIAL PERFORMANCE TIME MINIMUM weighting: 40%
