SUMMARY DESCRIPTION OF REQUIREMENTS AND PLACE OF DELIVERY
The purpose of this invitation to tender is to conclude a contract for the services of a SUPPLIER for the procurement of injectable anti-Rho(D) immunoglobulin (anti-D (human) IgG).
SITE VISIT
The PUBLIC ORGANIZATION is not planning a site visit.
TERM OF CONTRACT
The term of the contract is three (3) years with two (2) options to extend for one (1) year each.
ALLOCATION
The PUBLIC ORGANIZATION plans to use the concept of allotment. Thus the Goods are divided into the following lots, according to the mode of administration :

Lot 1 Good intramuscularly;
Batch 2 Good intravenous;

SECURITY REQUIREMENTS
No bid or performance security is required for this tender.
BID SUBMISSION
The SUBMISSIONER may submit its bid electronically in the OSAP or in hard copy to the address indicated in the tender documents.
CONDITIONS AND ELIGIBILITY CRITERIA

To be eligible to bid, a SUBMISSIONER :

must possess the necessary qualifications, authorizations, permits, licences, registrations, certificates, accreditations and attestations;
must have, in Quebec or in any other territory, including one not covered by an applicable Intergovernmental Agreement, an establishment where it carries on its activities on a permanent basis, clearly identified in its name and accessible during normal business hours;
having an establishment in Quebec must hold, at the time of filing its Submission, a valid attestation from Revenu Québec that was not issued after the deadline for receipt of Submissions;
not having an establishment in Québec must submit, with its Submission, the duly completed and signed "Absence of Establishment in Québec" form;
must not, at the time of submitting its Submission, be registered on the Register of Enterprises Ineligible for Government Contracts (RENA) or, if registered, be in a period of ineligibility;


The Register of Ineligible Enterprises (RENA) can be consulted on the website of the Public Procurement Authority at the following address: https://amp.gouv.qc.ca/rena/ . For any additional information concerning the INER, contact the GPA by telephone at 1 888 335-5550 . For more information about the register of ineligible enterprises, refer to sections 21.1 and following of the Act respecting contracts of public bodies (RLRQ, c. C-65.1) .


must have submitted with its Submission the "Bidder's Certificate of Probity" form duly completed and signed;
must have submitted with its Submission the "Declaration Concerning Lobbying Activities" form duly completed and signed;
must provide with its Submission the monograph of the proposed Property(ies) as indicated in section 3 c) of the Specifications;
must provide with its Submission any documentation regarding the DIN allocation of the proposed Good(s) as indicated in section 2 c) of the Specifications;
must provide with its Submission the letter of authorization from the Ministère de la Santé et des Services sociaux (MSSS) for the proposed Good(s), as indicated in section 2.2 b) of the Specifications;
must provide with its Submission an example of the labels that will be affixed to the box and vials of the proposed Property(ies) and on which a barcode/GS1 code appears, as indicated in section 3 a) of the Specifications;


must provide with its Submission the valid Notice of Compliance issued by Health Canada for the proposed Good(s), as indicated in section 2.1 (d) of the Specification;


must provide with its Submission its Manufacturer's and Importer's Establishment Licence, as indicated in section 2.2 (a) of the Quotation;


must meet, if applicable, any other eligibility requirements specified in the Bidding Documents.

A SUBMISSIONER's failure to comply with any of these conditions shall render him/her ineligible.
