The subject of the contract is successive delivery of reagents for identification and determination of drug susceptibility of microorganisms together with lease of the analyzer for the Department of Medical Laboratory Diagnostics of the Regional Specialist Complex in Rzeszów: 1) Price form2) Requirements for media and reagents;3) Summary of parameters required for analyzer for identification and determination of drug susceptibility of microorganisms4) Draft provisions of the public procurement contract - supply agreement5) Draft provisions of the public procurement contract - lease agreement- which are attached to the SWZ and available in separate files.
The subject matter of the contract is successive delivery of reagents for identification and determination of drug susceptibility of microorganisms together with lease of an analyzer for the Department of Medical Laboratory Diagnostics of the Regional Specialist Complex in Rzeszów2 .The Ordering Party stipulates that the order shall be executed according to the Ordering Party's current demand, up to the amount of funds secured in the contract, which means that during the term of the contract the Ordering Party may order less goods than those indicated in the bid, but it shall not be less than 60% of the contract value, and the Contractor shall not be entitled to any claims against the Ordering Party in such a case.3.The subject matter of the contract must be of current production and have all legally required approvals and certificates permitting its marketing in the territory of the Republic of Poland, which the Contractor undertakes to present at each request of the Principal. The item supplied by the Contractor, must have certificates of approval for marketing.4. The offered assortment which is a medical device must be approved for marketing and use and have current documents in accordance with the Act of 20 May 2010 on medical devices (i.e. Journal of Laws of 2017, Note: The Contractor with whom the contract will be signed will make available to the Purchaser photocopies of the relevant documents resulting from the regulations, requirements specified above at his every request.6. the subject of the contract offered by the Contractor must be consistent with the specified, complete and after assembly, installation, setting up ready to work without any additional purchases and investments.
