Description

The subject of the order is the delivery of 3 Type C ambulances for the needs of the Radom Emergency Ambulance Station in Radom with parameters consistent with the requirements described in the detailed description of the subject of the order, Parameters form required in Appendix 2 to the ToR. The offered ambulances are to be made on a van-type vehicle and must have identical commercial and technical and operational features.
The subject of the order is the delivery of 3 Type C ambulances for the needs of the Radom Emergency Ambulance Station in Radom with parameters consistent with the requirements described in the detailed description of the subject of the order, Parameters form required in Appendix 2 to the ToR.2 The offered ambulances are to be made on a van-type vehicle and must have identical commercial, technical and operational features.2Requirements concerning the subject matter of the order2.1 Offered ambulances must have brand new equipment, free from defects, covered by the vehicle manufacturer's warranty, meeting the requirements of the Road Traffic Law of 20 June 1997 (i.e. Journal of Laws 2020 item 110 as amended), Regulation of the Minister of Infrastructure of 31 December 2002. on the technical conditions of vehicles and the scope of their necessary equipment (i.e. Journal of Laws 2013, item 951), the applicable standards PN EN 1789 +A2, or equivalent for a C type of road transport vehicle, with a medical section, meeting the requirements of the harmonised Polish standard PN EN 1789 +A2 or equivalent required by the NFZ.2.2 The minimum requirements for the base car and medical compartment, as well as medical equipment, are contained in the detailed description of the subject matter of the order, the form of parameters required - Appendix No. 2 to the ToR2.3 The offered ambulances must have a certificate of approval of a completed vehicle (base car with medical equipment), issued in accordance with the Regulation of the Minister of Transport, Construction and Maritime Economy of 25 March 2013. on type approval of vehicles and trailers and their equipment or parts (Journal of Laws of 2015, item 1475), or unit approval, Test Summary and Certificate of Conformity in accordance with PN EN 1789+A2, or equivalent. The approval / unit approval of the offered ambulance must enable registration in Poland.2.4 The offered equipment must have the documents permitting its marketing and use in Poland and meet the requirements of the Medical Devices Act of May 20, 2010 (Journal of Laws of 2020, item 186, as amended).2.5 The delivered ambulances should be new (manufactured not earlier than in 2020)and not used, without defects, construction, executive and other technical defects that could become apparent during its use.2.6 The Contractor shall provide full warranty and post-warranty service for the offered ambulances, as well as full warranty service of the base vehicle at an Authorized Service Station.3 The following minimum warranty periods are required, counted from the date of signing the delivery and acceptance protocol by the parties.3.1 mechanical warranty for the base vehicle - min. 24 months without kilometer limit; 3.2 paint coatings - min. 24 months;3.3 corrosive perforation of body elements - min. 120 months;3.4 for medical bodywork - min. 24 months, warranty inspections free of charge every 12 months - 2 inspections in the period of 2 years, including all costs of ambulance transport to the indicated warranty service.3.5 Execution of reported defects and warranty repairs, during the warranty period - max. within 72 hours from the notification, at the expense of the guarantor. The Customer allows the replacement of the equipment with replacement parameters equal or better in case of repair above 72 hours.CPV codes: 34114121-3 - ambulances4. A detailed description of the subject matter of the contract is provided in Attachment No. 2, and detailed terms and conditions of contract performance are specified in the specimen Agreement constituting Attachment No. 6 to To ToR.5 The Awarding Entity requests indication of the parts of the contract whose performance the contractor intends to entrust to a subcontractor and the names of subcontractors (companies) if they are already known to the contractor at the tender submission stage.6 The Awarding Entity does not reserve the obligation to perform the contract personally by the contractor.7 Place of contract performance: NUTS code: EN 921,
