The Department of Veterans Affairs, Network Contracting Office 10, located at 24 Frank Lloyd Wright Drive, Lobby M, Suite 2200 in Ann Arbor, MI 48105, intends to post a solicitation for 5/8 Dry Ice Pellet Delivery Service for the Research Department located in Building 31 at the Ann Arbor VA Medical Center, 2215 Fuller Road, Ann Arbor, MI 48105. The solicitation will expire 05/06/2019 at 8:00am EST. The period of performance shall be Base Year 05/13/2019-05/12/2020 and for Four (4) One (1) Year Options 05/13/2020-05/12/2024 in strict accordance with terms, conditions and provisions of the contract solicitation, NAICS Code 325120. This solicitation is open to all small business vendors

This is a follow-on to 36C25019Q0638.

Statement of Work

The Contractor shall provide all necessary labor, materials and transportation as necessary to deliver 5/8 dry ice pellets to Building 31 at the Ann Arbor VA Medical Center located at 2215 Fuller Road, Ann Arbor, MI 48105.

Description of items (s):

Deliver 50 pounds of 5/8 dry ice pellets every Monday.

Deliver 50 pounds of 5/8 dry ice pellets every Wednesday.

Deliver 50 pounds of 5/8 dry ice pellets every Friday.

Transportation: transportation via refrigerated truck shall be included.

Delivery:

Location: deliveries shall be made to building 31 of 2215 Fuller Road, Ann Arbor, MI, 48105. Contractors will be met at the building entry and escorted by research staff to the appropriate room.

Receiving hours: all deliveries shall be made between the hours of 8:00 a.m. and 9:30 a.m. Monday, Wednesday and Friday, excluding holidays and any day declared an official federal holiday. Holidays observed by the VA Medical Center are:

Martin Luther King Day Columbus Day

President s Day Veteran s Day

Memorial Day Thanksgiving Day

Independence Day Christmas Day

If a government holiday falls on a Monday, then scheduled delivery will be on Tuesday of that week. If a government holiday falls on a Wednesday, then scheduled delivery will be on Tuesday of that week. If a government holiday falls on a Friday, then scheduled delivery will be on Thursday of that week.

VA has the option to increase, decrease, or cancel any delivery. Contractor shall receive one (1) day notice in any of these situations.

Safety requirements: In the performance of this contract, the contractor shall take such safety precautions as the Contracting Officer or his designee may determine it be reasonably necessary to protect the lives and health of occupants of the building.
