This is a combined synopsis/solicitation for commercial supplies prepared in accordance with the format in Subpart 12.6, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quotes under solicitation number: 36C26219Q1294.

The Department of Veterans Affairs Long Beach Healthcare System, Long Beach, California 90822 has a requirement to provide hazardous compounded chemotherapy drugs for VA Medical Center Long Beach, Pharmacy Service.

VA LONG BEACH HEALTHCARE SYSTEM:

Delivery Location Pharmacy Service, Bldg. 164

Normal Work Hours 7:00am to 4:00pm, Monday through Saturday

There will be approximately (30) thirty patients on any given day, Monday through Friday for chemotherapy infusion for 23 weeks.  Below is a list of the common chemotherapy drugs will be provided directly to the vendor. Must be able to supply and deliver all hazardous compounded chemotherapy drugs listed below. Orders are based on need for each patient. Please include the temperature logger in shipments to monitor temperature of drugs for quality assurance. Shipping fees must be inclusive (cost per week) within the hazardous compounded chemotherapy drug.

The period of performance is October 1, 2019 to September 30, 2020.

The solicitation will be in accordance with FAR Part 12, 13, and 19, Acquisition of Commercial Items, Simplified Acquisition Procedures, Small Business Programs, and Veteran Affairs Acquisition Regulations as supplemented with additional information in this notice. The North American Industrial Classification System (NAICS) code for this requirement is 325412 size standard 1250 employees. The Contractor shall adhere to all federal and state laws and regulations in effect during the term of this contract. The Government intends to award a firm-fixed-price contract. The Contract begins on the date of award. DUE TO TIME RESTRAINTS, responses to the solicitation shall be due on September 17, 2019 at 12:00 NOON PST. NO EXTRA TIME WILL BE GIVEN. Offerors are advised that it is your responsibility to review and monitor the website frequently to ensure you have the most up to date information, including amendments.

Must be registered in System for Award Management upon bid of this request for quote.

Notice: No remanufactures or gray market items/ supplies will not be acceptable. Interested parties of RFI/RFQ must be an authorized reseller, distributor, or dealer of the O.E.M. (Original Equipment Manufacturer). Verification can be provided by an authorization letter or other documents from the OEM. Equipment/Supplies shall be in accordance with the OEM terms and conditions. No exceptions, must be a manufacturer authorized distributor.
