SMITH AND NEPHEW ARTHROSCOPIC INSTRUMENTS

(I)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this action. Quotes are being requested, and a written solicitation will not be issued.

(II)This solicitation is issued as a Request for Quote (RFQ). Submit written quotes on RFQ 36C24618Q9570

(III)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-98.

(IV) Any amendments issued to this combined/synopsis will ONLY be available on the FBO website (www.fbo.gov).

(V) The Network Contracting Office 6-SAO East intends to award a firm-fixed price contract for the VAMC SALISBURY 27705-3897.

(VI) The North American Industrial Classification System (NAICS) code for this procurement is 334510 with a small business size standard of 1250 employees. This procurement is a set-aside for SDVOSB and all eligible offerors may submit quotes. All offerors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award of a federal contract. The record must be complete and not missing elements such as representations and certifications. All SDVOSB s must be listed and registered in Vetbiz.gov to be considered for award.

(VII) Questions concerning this solicitation shall be addressed to Audrey Wooten Contract Specialist, and emailed to Audrey.Wooten@va.gov. All questions or inquires must be submitted no later than 10am Eastern Standard time on 8/6/2018. No phone call will be accepted.

(VIII) The Government will award a contract resulting from this RFQ to the responsible offeror whose quote is the lowest price technically acceptable. Quotes are due by 11AM. Eastern Standard time on 8/7/2018 Quotes received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

Brand Name Only: The medical center requires the instruments to provide care to the patient in the operating room. The instruments are an addition to the current sets and completion of sets same brand in use by the medical center. The standardization of instruments is necessary for continuity of care across the medical center, and reduces the chances of incorrect set assembly.

Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Brand Name only is being requested for this procurement for standardization. The medical center requires the instruments to provide care to the patient in the operating room. The instruments are an addition to the current sets and completion of sets same brand in use by the medical center. The standardization of instruments is necessary for continuity of care across the medical center, and reduces the chances of incorrect set assembly. The medical center owns their equipment and stocks disposables that only brand name is compatible. It is in the best interest of the government to purchase brand name to prevent additional capital and accessory stock.

Description of market research conducted and results or statement why it was not conducted: Market research in accordance with FAR PART 10 was conducted) utilizing the World Wide Web, Google, VIP, FBO and GSA. Brand Name Only is being requested for standardization.

Addendum to 52.212-2, Basis for Award

Offers will be evaluated on the basis of Lowest Priced Technically Acceptable. The evaluation process shall proceed as follows.

1. Initially, offers shall be rank according to price, including option prices if applicable. An offeror s proposed price will be determined by multiplying the quantities identified in the pricing schedule by the proposed unit price for each contract line item to confirm the extended amount for each item. The extended amounts will be added together to determine the total evaluated price.

2. Next, the lowest priced offer shall be evaluated for technical acceptability. This is a Brand Name Only requirement. Technical acceptability shall be evaluated on a pass/fail basis considering whether the proposed product meets or exceeds the salient characteristics of the PRICE SCHEDULE.

3. If the lowest priced offer is determined technically acceptable, that offer represents the best value to the government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers.

4. If the lowest priced offer is not determined technically acceptable, the next lowest priced offer will be evaluated for technical acceptability and the process will continue in order by price until an offer is judged technically acceptable
