Description

The subject of the contract is: purchase and delivery of diagnostic X-ray apparatus with assembly and installation - 1 set for the Katowice Oncology Centre in accordance with Appendix No. 2 to the ToR - price offer. Appendix No. 3 to the ToR specifies the technical and functional parameters of the equipment.The contract also includes:1) transport of medical equipment to its destination,2) installation and installation,3) cooperation with the room adaptation contractor and CT scanner supplier in the location,4) connection of the supplied equipment to the Ordering Party's PACS/RIS system and configuration for proper operation with this system (the equipment is to download data from DICOM Worklist issued by the PACS/RIS system and send the tests to the PACS system; equipment sends MPPS messages to the PACS/RIS system)5) personnel training for 5 persons in the use and practical use of medical equipment, after prior written agreement with the Employer ending with signing the "Training Protocol",6) transfer of medical equipment along with certificates,7) obtaining the necessary permits for the Employer to use the subject of the contract, including a permit from the State Provincial Sanitary Inspector in Katowice for the use of the subject of the contract.89) 24-month guarantee counted from the date of signing by authorized representatives of the Contracting Authority and the Economic Operator the "Protocol on faultless acceptance of medical equipment" and 36-month post-warranty service counted from the expiry of the guarantee period, under which, in accordance with the offer, the Economic Operator ensures the efficiency of medical equipment, includinga) service inspections as required by the manufacturer, b) repair and replacement of parts for brand new ones except when the failure is caused by improper operation of medical equipment, c) all repairs, inspections, maintenance, including spare parts and materials necessary for their performance, d) prolongation of warranty for the period of failure of medical equipment, in the case of warranty repairs, e) ensure that the total failure of medical equipment (in the case of failure) does not exceed 20 working days in 12-month periods, while the failure is the lack of efficiency of medical equipment making it impossible to perform all procedures with its use, 10) delivery of brand new, unused medical equipment with the year of manufacture 2019.
