The subject of the contract is: collection of municipal waste from the owners of real estate residing in the Brwinów municipality. The municipal waste collection system does not include waste generated as a result of conducting business activity.2. The service of collecting municipal waste produced and collected by the owners of the properties where the residents live includes waste: - mixed municipal waste with ash, - biodegradable municipal waste, - plastics, - multi-material packaging, - metal, - glass, - paper and cardboard, - used tyres, - furniture and other bulky waste, - used batteries, - chemicals (packaging of plant protection products, paints, adhesives, etc.).Outdated medicines, - construction and demolition waste from repairs.

Pursuant to art. 29 sec. 3a of the PPL Act, the Contracting Authority shall specify the requirements for the Contractor or Subcontractor to employ, on the basis of an employment contract, persons performing the indicated activities within the scope of contract performance, persons driving vehicles performing waste collection service.4Prior to commencing the construction works and not later than within 5 days from concluding the contract, the Economic Operator is obliged to submit to the Contracting Authority: a) a statement of the Economic Operator or Subcontractor on employment under the employment contract of persons performing the activities referred to in item 3; b) copies of employment contracts of persons performing the activities referred to in item 3, certified as true copies by the Economic Operator or Subcontractor respectively (together with a document regulating the scope of responsibilities, if any), or other documents.5Documents constituting evidence of employment for the employment contract of the above mentioned persons may be presented in the form of anonymized copies, in a manner ensuring protection of personal data of the employees, in accordance with the regulations on personal data protection (i.e. especially without addresses, PESEL number of employees, etc.). The information such as: name and surname of the employee is not subject to anonymization, date of contract conclusion, type of employment contract and the size of the job should be identifiable.6During the contract execution, at each request of the Ordering Party within the time limit specified in this request, the Economic Operator shall submit to the Ordering Party documents which constitute evidence of employment of the above mentioned persons for the contract of employment: a) referred to in item 1.2. a) referred to in item 4; b) a certificate from a relevant branch of the Social Insurance Institution confirming payment by the Economic Operator or sub-contractor of social and health insurance contributions for employment under the employment contract for the last settlement period; c) a copy of the proof, certified as true copies by the Economic Operator or sub-contractor respectively, confirming that the employee has been declared for insurance by the employer, anonymized in a manner ensuring protection of personal data of the employees, in accordance with the provisions on personal data protection. The name and surname of the employee shall not be anonymized.
