Poland - Ultrasound equipment

Description

The purchase of medical equipment - ultrasound equipment, X-ray equipment with C-arm, patient monitors with central station. A detailed description of the subject matter of the contract is given in Appendix 1a and 2 to the SWZ (Terms of Reference).
USG apparatus. Parameters in accordance with Attachment No. 2 to the SWZ.Subject evidence: 1. the Contracting Authority will require the following subject matter evidence to be submitted with the tender:a) Annex No. 2 - Summary of parameters and conditions required;b) Statement that the equipment offered in the tender is CE marked, meets the essential requirements and may be marketed and used in the territory of Poland - in accordance with the Act of 20.c) Documents confirming that the equipment offered in the tender is CE marked, meets the essential requirements and can be marketed and used in the territory of Poland - in accordance with the Act of 20 May 2010 on medical devices (Journal of Laws of 2021, item 1565). Documents prepared in a foreign language must be submitted with a translation into Polish, certified by the Economic Operator.d) Description or folder, together with a photograph of the equipment offered in the tender, based on materials from the manufacturer (e.g. catalog).
