1) The subject matter of the contract is providing the Ordering Party with E, F and G services; 2) the subject matter of the contract is divided into 7 parts The subject matter of the contract is divided into 7 parts:1) parts 3 and 5 cover the provision of service G;2) parts 1, 2, 6 and 7 cover the provision of service F and G;3) part 4 covers the provision of service E and G.
The subject matter of the contract is to provide the Contracting Authority with services F and G.Course no.: course no. PC 120Maximum quantities of services that can be commissioned to the Contractor by the Contracting Authority within the performance of the contract: Service F:- maximum number of pcs with option right (100%) - 508,- number of pcs F service: - maximum number of items with right of option (100%) - 508,- number of items (70%) - 356,- number of items covered by the right of option (30%) - 152,- G service: - maximum number of kilometres with right of option (100%) - 23,622,- number of kilometres (80%) - 18,898,- number of kilometres covered by the right of option (20%) - 4,724,2. Conditions under which the above mentioned sole source contract will be awarded:1) The scope of above mentioned contract will consist in repetition of services from the base contract and will include the following number of services (70 %) - 353,- number of units under option right (30 %) - 151,Service G: - maximum number of kilometres under option right (100 %) - 23 436,- number of kilometres (80 %) - 18 749,- number of kilometres under option right (20 %) - 4 687,2) A draft contract will be handed over to the Contractor together with the invitation to negotiate.3) Contractual penalties will be provided for in the same cases as in the contract concluded in the course of the basic procurement procedure;4) The obligations of the Economic Operator and the Awarding Entity will be governed by the same rules as in the contract concluded in the course of the basic procurement procedure;5) Contract period: up to 12 months.3If during the term of the agreement the Contractor's remuneration does not reach the maximum remuneration, the Parties may agree to extend the term of the agreement by a maximum of 3 months, but not longer than until the amount of remuneration for the services performed by the Contractor reaches the maximum remuneration. The extension of the agreement validity period according to the previous sentence requires a written form under pain of nullity. 4. 4. In the case referred to in item 3 the amount of the remuneration shall be amended in the event of a change of: 1) the rate of the goods and services tax and the excise duty,2) the amount of the minimum remuneration for work or the amount of the minimum hourly rate, established pursuant to the provisions of the Act of 10 October 2002 3) the principles of being subject to social or health insurance, or the amount of a social or health insurance contribution rate,4) the principles of collecting and the amount of payments to employee capital plans referred to in the Act of 4 October 2018 on employee capital plans - if such changes affect the costs of contract performance by the Economic Operator.5) costs related to the execution of the order with respect to the price of diesel oil on the terms specified in the draft agreement.
