Description

As a result of the SGB V, the Association of Statutory Health Insurance Physicians performs the tasks assigned to it in the interest of physicians and psychotherapists working in private practice. In addition to a wide range of activities, it is the task of the KVH to organize a medical on-call service and to ensure that it functions properly. In recent years, this medical on-call service has developed from an obligatory offer within the framework of the guarantee mandate outside of practice opening hours to a comprehensive service offer oriented to the needs of the population. When calling 116 117, Arztruf Hamburg offers contact to medical specialists who carry out an initial medical assessment by telephone by means of a nationally uniform system-supported query SmED (structured initial medical assessment in Germany, https://www.zi.de/smed). SmED is a product in accordance with the German Medical Devices Act (MPG), for which the user receives intensive training. The initial assessment result provides a recommendation on the urgency of treatment as well as the location of treatment. Depending on the result, the different service areas of the physician call Hamburg are triggered or the switching to the emergency call 112 is carried out.The visiting service for the ambulatory family doctor emergency care of the population clearly distinguishes itself from the rescue service, which is carried out by the fire department and the relief organizations, as well as private companies. There are points of contact between the Arztruf Hamburg and the rescue service, which can result from the corresponding procedures of the respective operation. In general, Arztruf Hamburg stops where the tasks of the rescue service begin. This is, among other things, the reason why the vehicles of Arztruf Hamburg do not have special signal systems.
Procurement objects:Call acceptance of the 116 117 in the night time between 23:45 o'clock and 07:15 o'clock, all year round Here the first contact of the patient with the physician call Hamburg comes off, if the entrance into the supply possibilities over the call number 116 117 takes place. The perception of the service is shaped by the call acceptance process. A contemporary greeting, a clearly structured, system-supported dialog query and friendly but leading communication are the core of call handling. The Kassenärztliche Vereinigung Hamburg (Hamburg Association of Statutory Health Insurance Physicians) ensures that employees are trained in the use of the SmED medical product. The employees employed in the switchboard have a basic medical qualification, from the areas of the rescue service (at least paramedic or equivalent qualification), the hospitals (nurse or intensive care nurse) or from medical practices (medical assistant).Disposition of the assignments of Arztruf Hamburg (patient visits), 24/7 all year round, workplace is located in the control center of the Kassenärztliche Vereinigung Hamburg The assignments created by the query algorithm SmED, in their corresponding prioritizations, lead to good utilization and use of resources through optimal disposition. The assessment of the dispatching performance is based on the criteria of arrival or waiting times in connection with the prioritization of the patient and in connection with the mileage of the vehicle fleet. Another factor influencing dispatching is a balanced distribution of assignments among the various resources. The employees deployed in dispatching must have in-depth knowledge of the Hamburg city area and be able to clearly assign districts and streets on the Hamburg city map. A distinctive disposition experience in similar environments (patient transport, courier services, goods deliveries or passenger transports) is required.Physician Assistance / Driving Staff The task of the physician assistance includes driving the vehicle, operating the technical equipment in the vehicle and, if requested by the physician, assisting in the care of the patients as well as the necessary documentation on site. For this purpose, mobile technical aids and basic medical equipment are available to the staff. The workflow is controlled by comprehensive software-based process control. The employees deployed in the physician assistance have at least the qualification as a paramedic according to the "Principles for the training of personnel in the rescue service of the Bund-Länderausschuss Rettungswesen or Ausschuss Rettungswesen" or a completed equivalent training.e.g. the paramedic service according to TIV or ATN 1001369 of the German Federal Armed Forces is considered equivalent. In the first six months of the contract period, it is also possible to use paramedics in training as paramedics in a weighting of 60% paramedics / 40% paramedics for the driving service. During the remaining contract period, a weighting of 70% paramedic / 30% rescue assistant is possible. The safe use of the various medical equipment is considered self-evident, especially in emergency situations.Fleet provision, operation and maintenance of the motor vehicles The current fleet size is given as 24 vehicles The vehicles should be uniform, identical in equipment and identifiable by specified labeling. A four-door compact vehicle with raised entry and seating level is required. Vehicles must have comprehensive driver and passenger safety equipment and be equipped with standard driver assistance systems. Features include leather upholstery for improved hygiene in conjunction with heated seats, a parking heater and Apple CarPlay or Android Auto and three powerful USB ports. The vehicles have a holder in the center console area for the mobile devices provided by KVH, which also provide sufficient attachment in the event of a crash. The tires must be appropriate for the season and therefore exist 8-times. Control of the vehicle fleet is the responsibility of the service provider, who is also responsible for compliance with the relevant repairs or maintenance and their deadlines. A QM system, e.g. certified according to DIN EN ISO 9001:2015, which guarantees vehicles that are always ready for use and roadworthy, must be provided by the service provider. The QM system also includes the forgery-proof documentation of the operating equipment and its recordings. The infrastructure of the vehicle fleet is characterized by an appropriate stock of spare parts and has performance and intervention times that ensure rapid repair of minor damage, e.g. due to parking damage, even outside normal workshop hours. A responsiveness 24/7 for the control center is required.Maintenance and care of medical equipment Each vehicle of Arztruf Hamburg is equipped with emergency medical equipment specially procured by the KVH, with which bridging immediate measures in the area of heart / circulation, control of cardiac activity, ventilation, securing and monitoring the oxygen supply, and wound care can be carried out. The emergency medical equipment is specified in detail by the KVH and is constantly being further developed in cooperation with the physicians working in the emergency service. The equipment is stored in a backpack specially designed for emergency services and can be easily taken to the patient by the driving personnel. The backpack also includes an AED (automatic external defibrillator) and a carrying bag with an oxygen unit. The maintenance, completion and care of this equipment provided by the Kassenärztliche Vereinigung Hamburg (Hamburg Association of Statutory Health Insurance Physicians) is the responsibility of the driving personnel, with the exception of the equipment that is subject to a safety technical inspection (STK). The performance of the STK and the procurement of consumables is the responsibility of the service provider, this also includes infection control clothing and disinfectants for hand disinfection. Invoicing takes place according to documentation and expenditure in coordination with the KVH.
