The subject of the contract is the provision of comprehensive services related to the supervision over the implementation of the agreement concluded as a result of a separate procedure for the task of the system to be implemented design and build the following: "Extension of the voivodship road No. 777 from DK 74 to DW 759 and construction of a new section of DW 759 from DW 777 to the border of the voivodship - stage I":- section: extension of the voivodship road No. 777 Maruszów-Piotrowice in km 21+513 to 26+332,- section: construction of the voivodship road No. 759 Piotrowice - gr. Scope of service: - management and coordination of investment implementation (design stage and construction works), - provision of investor supervision services, - performance of reporting duties. Detailed data on the scope of the service is included in the detailed technical specification (SST).
The subject of the contract is to provide comprehensive services related to the supervision of the implementation of the contract concluded as a result of a separate procedure for the task of the implemented system design and build the system called "design and build".Extension of voivodship road no. 777 from DK 74 to DW 759 and construction of a new section of DW 759 from DW 777 to the border of the voivodship - stage I": - section: extension of voivodship road no. 777 Maruszów-Piotrowice in km 21+513 to 26+332, - section: construction of voivodship road no. 759 Piotrowice - gr. of the voivodship in km 0+000 to km about 2+750.General characteristics of the investment: The investment task under supervision is carried out in the "design and build" system. The scope of the investment task under supervision includes: a) development of the project documentation under the Functional and Utility Program (PFU) together with the necessary administrative decisions, opinions and arrangements allowing for carrying out construction works for the investment task, including the environmental decision and water law permit for the construction of water facilities, demolition.Within the framework of the contract (in the contract price and time for completion), if necessary, it should also be included:- report and carry out the environmental impact assessment, - obtain, if necessary, deviations from the technical conditions;b) execution of construction works together with the necessary accompanying infrastructure on the basis of design documentation executed by the Economic Operator, together with services not being construction works and ensuring natural supervision in the scope resulting from special regulations and decisions on environmental conditions of the project, if necessary securing and relocating the monuments of small architecture, ensuring archaeological supervision and conducting rescue archaeological research, advance archaeological research (advance excavation research), ensuring sapper's and geological supervision. The detailed scope of works is presented in the PFU; c) exercising author's supervision over the developed design documentation; d) obtaining a permit for use.The investment should be designed and executed:- extension of DW 777 along the existing trail with possible correction of non-standard curves and construction of a 2.00 m wide asphalt pavement along DW 777 in the area of the village from behind the trench or in case of impossibility at the edge of the 2.5 m wide roadway with reconstruction of crossings and road culverts; extension of intersection DW 759 and DW 777 in km 26+311 21+513 with separation of turnings; bus bays, culverts, road drainage, construction/reconstruction of road lighting in accordance with applicable regulations, reconstruction of conflicting facilities and the existing infrastructure network under and above ground, felling of conflicting trees,- construction of a new course of DW 759 of approx. 1.50 km with the connection to the footprint of the intersection existing in km 2+250 with the gmina road no. 401018T. Extension of road no. 759 at the section from the crossing to the ferry crossing in km 2+610, approx. 0.36 km long, i.e. entry to the dike. Construction of a 2.00 m wide asphalt road along DW 759 in the area of the village situated behind a trench or in case of impossibility at the edge of the 2.5 m wide roadway, together with reconstruction of crossings and replacement of road culverts; demolition of the colliding buildings; incorporation of the DW 759 old road in the newly constructed section of the provincial road by extending the section of the commune road 401059T and construction of the crossroads of the municipal road 401059T with the new course of DW 759; reconstruction of the conflicting facilities and network of the existing infrastructure under and overground, reconstruction of crossroads, road culverts and drainage, construction/reconstruction of road lighting in accordance with the applicable regulations, cutting down of the conflicting trees.The Contractor is obliged to execute the order in accordance with the specimen contract and SST.
