Description

The object of the contract is to provide the Provincial Hospital in Bielsko-Biała, located at Al. Armii Krajowej 101, 43-316 Bielsko-Biała, within the period of 24 months, including: 1) rendering sanitary transport services of patients for the Provincial Hospital in Bielsko-Biała - Part 1,2) rendering sanitary transport services of biological material, cytostatic drugs and the Ordering Party's personnel for the Provincial Hospital in Bielsko-Biała - Part 2,3) rendering transport services of patients for haemodialysis procedures and kidney transplantation for the Provincial Hospital in Bielsko-Biała - Part 3.2) Detailed scope of the subject matter of the contract is defined as follows: 1) description of the subject matter of the contract constituting Attachment No. 2 to the Terms of Reference, hereinafter referred to as "STP", 2) draft provisions of the contract for each part of the contract constituting Attachments No. 6.1-6.3 to STP.3The subject matter of the contract shall be performed at the expense and risk of the Contractor. 1.
The subject matter of the contract is to provide the Provincial Hospital in Bielsko-Biała, 101 Armii Krajowej Avenue, 43-316 Bielsko-Biała, with sanitary transport services for patients, hereinafter referred to as "transport".2 The Contractor shall provide the service referred to in section 1 on a daily basis around the clock.3 Transport shall take place in the following areas:1) in the Bielsko-Biała and Bielsko-Biała counties, in particular on the route: Provincial Hospital in Bielsko-Biała, 101 Armii Krajowej Avenue - patient's place of stay/residence or medical facility. Estimated monthly number of transports: 300.2) on the territory of the districts: żywiecki, pszczyński, oświęcimski and cieszyński in particular on the route: Provincial Hospital in Bielsko-Biała Al. Armii Krajowej 101 - patient's place of stay/residence or medical facility. Estimated number of transports per month: 50.3) on the territory of the districts of the City of Bielsko-Biała and Bielsko-Biała in particular on the route: place of stay/residence of the patient - Provincial Hospital in Bielsko-Biała Al. Armii Krajowej 101 and return of the patient to the place of stay/residence of the patient after providing medical services (estimated time of providing medical services 1 hour). The Contractor must ensure that the patient's guardian/assistant can be transported with the patient. Estimated monthly number of transports: 100.4) on the territory of the districts: żywiecki, pszczyński, oświęcimski and cieszyński in particular on the route: the patient's place of stay/residence - Provincial Hospital in Bielsko-Biała Al. Armii Krajowej 101 and return of the patient to the patient's place of stay/residence after providing medical services (estimated time of providing medical services 1 hour), the Contractor must ensure the possibility of transporting with the patient the patient's guardian/companion. Estimated number of transports per month: 10.5) on the territory of the Republic of Poland excluding the counties of the City of Bielsko-Biała, Bielsko-Biała, Żywiec, Pszczyna, Oświęcim and Cieszyn in particular on the route: Provincial Hospital in Bielsko-Biała Al. Armii Krajowej 101 - patient's place of stay/residence or medical facility and Provincial Hospital in Bielsko-Biała Al. Armii Krajowej 101 - medical facility and return of the patient to Provincial Hospital in Bielsko-Biała Al. Armii Krajowej 101 after providing medical services (estimated time of providing medical services 1 hour). The Contractor must ensure the possibility of transporting the patient's chaperone/companion along with the patient. Estimated number of kilometers per month: 1,000 km. The Contractor shall be obliged to perform transport using the shortest route.4Each transport must be carried out by at least a basic medical rescue team within the meaning of art. 36 section 1 point 2 of the Act of 8 September 2006 on State Medical Rescue Service (i.e. Journal of Laws of 2021, item 2053 as amended).5. Detailed requirements concerning the means of transport to be used in the performance of the contract are specified in Appendix 2 to Terms of Reference, Section II, item 56. The Contractor shall maintain the vehicles used in the performance of the contract in a good technical and sanitary condition, allowing the Contracting Authority at any time to check their condition.7.The Contractor shall disinfect the vehicles used in the performance of the contract at least once a week and after transporting a patient diagnosed with an infectious disease. Disinfecting operations must be confirmed in writing.8Conditions of contract execution - see Appendix No. 2 to the Terms of Reference, Section II, paragraph 8 1. The subject matter of the contract is to provide the Voivodship Hospital in Bielsko-Biała, 101 Armii Krajowej Avenue, 43-316 Bielsko-Biała, with sanitary transport services of biological material, cytostatic drugs and the Ordering Party's personnel, hereinafter referred to as "transport". 2.The transport referred to in paragraph 1 includes: 1) sanitary transport of biological material, including blood and blood products; 2) sanitary transport of cytostatic drugs; 3) transport of doctor/nurse as part of outpatient night and holiday healthcare services provided by the Hospital. The transport will take place on the territory of the Republic of Poland, in particular in the provinces of Silesia and Malopolska. Estimated monthly number of kilometers: 3,000 km (average of 30 transports per month).5. The Contractor is obliged to perform transport by the shortest route.6. Detailed requirements for the means of transport intended for the execution of the order:1) the means of sanitary transport intended for the execution of the order must have the technical and quality characteristics specified in the Polish Standards carrying the European harmonized standards;2) each vehicle must have a Permit of the Minister of Internal Affairs and Administration for the use of motor vehicles as privileged in road traffic, referred to in Article 53, paragraph 1, item. 12 of the Traffic Law Act of 20 June 1997 (i.e. Journal of Laws of 2021, item 450, as amended);3) each vehicle must be manufactured no earlier than in 201544) each vehicle must be equipped with one transport refrigerator with a possibility of being plugged into the power socket in the vehicle, ensuring temperature in the min. 2-240C, equipped with two independent temperature gauges and a temperature regulator and one transport refrigerator with the possibility of plugging in to the power socket in the car, ensuring the temperature below -180C, equipped with two independent temperature gauges and a temperature regulator;5) each vehicle must be equipped with air conditioning,6) each vehicle must meet the requirements specified in the act - the law on road traffic and executive regulations;7) the ambulance driver must meet the requirements specified in art. 106(1) of the Act of 5 January 2011 on driving vehicles (i.e. Journal of Laws of 2021, item 1212, as amended).8) the contractor must have accident insurance for the number of persons consistent with the registration certificate of each vehicle (driver and passengers included).7. Requirements for sanitary transport of biological material, including blood and blood products and cytostatic drugs CBC and its components from 2 - 10°C, FFP and cryoprecipitate below -18°C, KKP from 20 - 24°C; cytostatic drugs from 2 - 8°C;2) Biological material, including blood and blood preparations and cytostatic drugs must be transported in a transport refrigerator or transport refrigerator, depending on the required transport temperature specified in point 1;3) Refrigeration equipment on the ambulance must be periodically validated at least once a year.8The Contractor is obliged to maintain the vehicles used to perform the contract in a good technical and sanitary condition, allowing the Purchaser to check this condition at any time.9. The Contractor is obliged to disinfect the vehicles used to perform the contract at least once a week. Disinfecting operations must be confirmed in writing.10. Conditions for the performance of the contract - see Appendix No. 2 to the Terms of Reference, Section III, paragraph 10 1. The subject matter of the contract is the provision of transport services for the Voivodship Hospital in Bielsko-Biała, 101 Armii Krajowej Avenue, 43-316 Bielsko-Biała, for patients undergoing haemodialysis treatments and kidney transplantation, hereinafter referred to as "transport", including: 1) significantly weakened and infirm walking persons;2) persons in wheelchairs - the estimated number of persons is 12 per day;3) persons requiring transport in the supine position on stretchers - the estimated number of persons is 3 per day.2Transport will take place as described in Appendix No. 2 to the Terms of Reference, Chapter IV, item 2. 3. Dates of arrival and departure of patients for hemodialysis treatments in the scope of transport referred to in paragraph 2 items 1-4: 1) 1st shift - arrival no later than 7:00 a.m.; departure after completed dialysis, taking into account the provisions of paragraph 4, 2) 2nd shift - arrival no later than 12:30 p.m.; departure after completed dialysis, taking into account the provisions of paragraph 4, 3) 3rd shift - arrival no later than 7:00 a.m. 4) 3rd shift - arrival no later than 6:30 pm; departure after completed dialysis, taking into account the provisions of para. 4,4) hours of arrival and departure for transportation referred to in para. 1 pt. 2 i.e. on Sunday will be determined on an individual basis, which will be immediately communicated to the Contractor by the responsible nurse of the Dialysis Station of the Voivodship Hospital in Bielsko-Biała.4The average duration of hemodialysis treatment is 4 hours, which may change depending on the patient's health condition. The maximum waiting time for a person to be transported is 80 minutes.5 Detailed times of patient arrival and departure for haemodialysis treatments shall be specified in monthly schedules prepared and updated by the Ordering Party.6 The Contractor shall strictly observe the patient arrival and departure times for haemodialysis treatments in order to ensure correct and uninterrupted operation of the Ordering Party's Dialysis Station.7 .The Contractor is obliged to make sure that each driver providing transport services has the so called "driver's card" containing at least the number of transports and pickups of patients for hemodialysis treatments on a given day, on which a unit nurse of the Dialysis Station of the Voivodship Hospital in Bielsko-Biała will confirm the number of transports and pickups of dialysis patients on the following day.8 .In case of a road accident involving dialysis patients being transported to and from Dialysis Station of the Regional Hospital in Bielsko-Biała and for kidney transplants: 1) the driver of the vehicle shall immediately call the emergency medical services and inform the Dialysis Station of the Regional Hospital in Bielsko-Biała about the accident; 2) the dialysis patients not qualified by the emergency medical services to be transported to hospital shall be provided with substitute transport by the Contractor.9The Contractor shall provide the service using means of transport, ensuring safe, hygienic and comfortable conditions of transport and meeting the requirements listed below:10 The Contractor shall maintain the vehicles used to perform the contract in a proper technical and sanitary condition, allowing the Ordering Party to check the condition at any time.11The service referred to in item 1 shall be rendered within Bielsko-Biała poviat, Bielsko poviat and a part adjacent to Bielsko poviat from Pszczyna, Cieszyn and Żywiec poviats.12. List of patients requiring transport for hemodialysis treatments, specifying their number, location of patients for hemodialysis treatments and type of transport - as at 31.03.2022: - Attachment No. 2 to the Terms of Reference, Chapter IV, paragraph 2.
