Description

1. delivery and installation of ICT infrastructure and technical architecture in the project partner's locations;2. configuration and start-up of the infrastructure in the locations indicated by the project partner;3. delivery and installation of software;4. granting licenses for the project partner;5. conducting trainings for administrators in the scope of administering the delivered hardware and software;6. 6. provision of warranty service - within the implementation of the project entitled "Introduction of modern e-services in medical entities supervised by the Minister of Health", within the scope and in accordance with the requirements of the Description of the Object of Procurement (OPZ), constituting appendices to the Terms of Reference (SWZ).
I.1 The deliveries, which are the subject of this order, carried out by the Contractor are intended to ensure that the objectives of the project "Implementation of modern e-services in medical entities supervised by the Minister of Health" are achieved, as measured by indicators defined in the project documentation, among others in the form of implementation and expansion of the e-services system in the project partners.2The aim of the project is to implement IT solutions assuming the improvement of availability, quality and effectiveness of performed healthcare services as well as the strengthening of organizational potential of healthcare entities supervised by the Minister of Health, and also to enable the integration of the aforementioned solutions with other ICT systems (IT infrastructure) used at project partners/entities and other healthcare IT systems providing data for the project's business services.3The recipients of products and results of the project shall be patients using medical services in Poland financed directly from social security contributions or other public funds, project partners and institutions supervising them.4 Detailed information on the implementation of this supply have been included in the Description of the Object of Procurement (OPZ), constituting an appendix to the Terms of Reference (SWZ).II.1Subject matter of the evidence - pursuant to provisions of art. 105 and 106 of the PPL Act in order to confirm compliance of the offered supplies with the requirements, features or criteria determined in the description of the subject matter of the contract or evaluation criteria or requirements related to contract performance the Contracting Authority demands from the Economic Operators to present the following documents:1.1. detailed technical documentation of the manufacturer of the offered hardware and software as required, and if any of the parameters are not indicated in the technical specification - also another document or a statement. The technical documentation and other aforementioned documents or statements shall be the basis for confirming compliance of technical parameters of the offered equipment or solutions with the parameters required in individual technical specifications. The Contracting Authority allows the submission of technical documentation in English without translation into Polish;1.2. a statement by the Contractor confirming that the company that will carry out servicing of the offered equipment has implemented a quality management system compliant with ISO 9001:2015 or equivalent for the provision of servicing services;1.3. A document confirming that the manufacturer of the offered equipment or solution has a certificate of compliance with the ISO 9001:2015 standard or equivalent in the field of quality management, as well as with other standards required for particular equipment and solutions - if listed in the OPZ and annexes to the OPZ;1.4. A certificate or declaration of conformity CE of the offered equipment;1.5. A statement of the Contractor that the offered equipment is covered by the equipment manufacturer's warranty for the period indicated in the Contractor's offer, and that the manufacturer's warranty services will be provided by the manufacturer or the manufacturer's authorized service in accordance with the requirements of the OPZ.2Pursuant to provisions of Article 107(1) of the PPL Act the Economic Operator shall submit the above mentioned means of evidence together with the tender.3 The Contracting Authority pursuant to Article 107(2) of the PPL Act shall allow the above mentioned means of evidence to be supplemented once upon the Contracting Authority's request.4 The documents referred to above shall be submitted in the manner prescribed in provisions issued pursuant to Article 70 of the PPL Act.5Documents prepared in a foreign language shall be submitted with a translation into Polish; however the Contracting Authority shall accept submission of the technical documentation referred to in item 1.1 and certificates or CE declaration of conformity of the offered equipment referred to in item 1.4 in English without translation into Polish.
