Description

The subject of the contract is "Implementation of EDM and e-services in Spec. Psych. ZOZ w Łodzi" for the needs of the project "Implementation of e-services and electronic medical records in the Specialist Psychiatric Health Care Team in Łódź". The subject matter of the contract includes: 1. electronic medical documentation, according to the requirements described in Appendix No. 1 to OPZ:1.1. e-services on-line, according to the requirements described in Appendix No. 2 to OPZ;1.2. training with EDM, according to the requirements described in Appendix No. 3 to OPZ; 2. within the scope of the services of configuration and installation of the delivered software, the Orderer requires: 2.1. preparation of a document Order Execution Plan, 2.2. installation and configuration of the supplied software, 2.3. reconfiguration of the existing data processing environment, 2.4. execution of as-built documentation. After completing the order, the Contractor will be obliged to provide warranty service and updating care for not less than 12 months.
The subject matter of the order includes granting a license to use the supplied software and implementation, understood as installation and configuration of software for electronic medical records (EDM) and electronic services (e-services) together with trainings and installation and configuration of the necessary tool software within the framework of the project financed from the European Union funds. This order is part of the project "Implementation of e-services and electronic medical records in the Specialist Psychiatric Health Care Team in Łódź" and is implemented with co-financing from the European Regional Development Fund within the framework of the project: Regional Operational Programme of the Łódzkie Voivodeship for the years 2014-2020, priority axis VII: Infrastructure for social services, measure VII.1 Information and communication technologies, sub-measure VII.1.2 Information and communication technologies.The subject of the contract is "Implementation of EDM and e-services in the Specialist Psychiatric Health Care Team in Łódź" for the needs of the project "Implementation of e-services and electronic medical documentation in the Specialist Psychiatric Health Care Team in Łódź".The subject of the contract includes:1. electronic medical documentation, according to the requirements described in Appendix No. 1 to OPZ:1.1. e-services on-line, according to the requirements described in Appendix No. 2 to OPZ;1.2. training with EDM, according to the requirements described in Appendix No. 3 to OPZ.2.Within the framework of the services of configuration and installation of the delivered software, the Contracting Authority requires:2.1. preparation of the Order Execution Plan document;2.2. installation and configuration of the delivered software;2.3. reconfiguration of the existing data processing environment;2.4. execution of the as-built documentation.2.4. after completion of the order, the Contractor shall be obliged to provide warranty service and updating care for a period not shorter than 12 months (according to the Contractor's offer) from the date of signing the Final Acceptance Protocol. The Contracting Authority indicates that the guarantee period is a scored parameter constituting the second criterion for evaluation of tenders.The functional requirements specified in Annexes 1 to 3 to the OPZ should be treated as minimum requirements.The Contracting Authority requires that the offered subject matter of the contract meets the requirements specified in the applicable Polish law as well as in the law of the European Union, has been admitted to commercial trading and has valid documents required by law, confirming the admission to use in Poland, and must meet at least other minimum requirements (standards, parameters) specified by the Contracting Authority in Annexes 1 to 3 to the OPZ.
