Description

1) Counselling and support:- Information mediation,- Making contact and initial conversation,- Anamnesis and diagnostics,- Indication preparation,- Continuous counselling interviews,- Problem-oriented counselling,- Motivational work,- Mediation in further help,- Initiation of self-help groups and cooperation,- Crisis intervention,- Work with reference persons,- Work with specific problem groups (eating disorders, migrants, smoking cessation),- Gender-specific offer (women's groups).2) Youth and drug counselling:- Target group-specific services.3) Outreach activities as home visits and streetwork:- Clients' outreach work in their homes,- Follow-up social work with CMA clients,- Cooperation with homeless help,- Continuous search for typical meeting points,- Establishing contacts with substance consumers,- Building trust,- Minimizing damage,- Providing information and individual case support,- Mediation in further assistance systems,- Cooperation with youth welfare services, in particular youth social work/continuation of street work services,- Outreach work in detoxification wards of local hospitals,- Counselling in structures of health care,- Counselling in prisons,- Outreach work in the field of migration and asylum seekers.4) Treatment/Rehabilitation:- Outpatient weaning treatment according to the criteria of the agreement "Abhängigkeitserkrankungen" of 04.05.2001 and the Gemeinsamen Rahmenkonzeptes der Deutschen Rentenversicherung und der Gesetzlichen Krankenkassen zur ambulanten medizinische Rehabilitation Abhängigkeitskranker of 3.12.2008,- Outpatient aftercare according to the criteria of the Gemeinsame Rahmenkonzeptes der Deutschen Rentenversicherung und der Gesetzlichen Krankenversicherung zur Nachsorge of 31.10.2012.5) Networking work:- Client-related cooperation with other services to optimise aid for clients,- Participation in case conferences and aid plan preparation,- Institutional cooperation with other services (participation in the psychosocial working group (PSAG) and special and supraregional working groups),- Participation in socio-political committees.6) Documentation:- IT-supported systematic client and activity recording (standard German core data set with module Brandenburg) with regular annual preparation of a structured factual report.7) Public relations:- Presentation of the addiction problem with reference to regional needs and development,- Presentation of the activities, goals and offer of the addiction counselling centre in public.8) Prevention:- cooperation with prevention experts,- contact persons and contact points for institutions, communities, multipliers,- project offers,- information offers,- implementation of model projects.9) Multiplier work:- Organisation of information events.10) Damage minimisation:- Crisis intervention - also outreach,- Open contact area and meeting place.11) Psychosocial substitution supportTarget groups: The benefits are aimed at persons who have a substance-related and substance-independent addiction disease, persons with risky or harmful patterns of consumption, as well as relatives and caregivers and possible cooperation partners. Scope of the benefit provision: The counselling service should be available 5 days a week from 9.00 a.m. to 4.00 p.m. in Wildau and in Lübben and at least 2 days a week in Luckau. During these times, advice centres can be contacted personally and also by telephone. The financing is carried out by means of an annual grant notice for the fixed amount financing as allocation from state funds and funds of the district.
