Description
The subject matter of the procedure and the contract is to select a Contractor for the supply of phantoms (training trainers) for the needs of the Centre for Innovative Medical Education of the Jagiellonian University - CM in Kraków, respectively for part 1 of the contract. The scope of the contract includes, in particular, purchase, transport, packaging, unloading, bringing in, commissioning at places indicated by the Contracting Authority, warranty service as well as provision of warranty services, in relation to part 1 of the contract. Within the scope of the agreed value of the order for part 1 of the subject of the order, the Contractor shall be obliged to conduct a 1-day training course on the use of the subject of the order (total duration of training: at least 2 clock hours) for 3 employees of the Ordering Party, i.e. Zakład Dydaktyki Medycznej UJ CM (Medical Education Department of the Jagiellonian University, CM) within the time and hours agreed with the User. A detailed description of the subject matter of the contract, specifying the functions, technical parameters and conditions required, or their maximum or minimum ranges or other necessary features, and additionally scored, is included in Appendix A to the Offer Form, which constitutes an integral part of the ToR. The description of the subject matter of the contract is presented in point. (3) the ToR and the model contract.
