The subject matter of the contract is collection and management (recovery or neutralisation) of municipal waste from residential properties and properties with no residents, where municipal waste is produced, located in the Town and Municipality of Glinojeck.
The subject matter of the contract is collection and management (recovery or neutralisation) of municipal waste from residential properties and from properties where no residents reside but where municipal waste is produced, located in the Town and Municipality of Glinojeck.In order to confirm that there are no grounds for exclusion from the procedure, the Economic Operator whose tender was awarded the highest mark will be called upon to submit the following subjective evidence, valid on the day of submission, referred to in the Regulation on subjective evidence and other documents or statements which the ordering party may demand from the economic operator (Journal of Laws of 2020, a) Information from the National Criminal Register, within the scope of art. 108 sec. 1 item 1, 2 and 4 of the Act - prepared not earlier than 6 months before submitting it - if the Economic Operator has a seat or place of residence beyond the borders of the Republic of Poland, instead of information from the National Criminal Register within the scope referred to in art. 108 sec. 1.1, 2 - shall submit information from a relevant register such as a court register, or, if there is no such register, another equivalent document issued by a competent judicial or administrative body of the country in which the Economic Operator has its registered office or place of residence;b) A statement confirming the validity of the information contained in the JEDZ statement within the scope of art. 108 item 1.3 and 4 of the Act.c) A statement confirming that the Economic Operator does not belong to the same capital group within the meaning of the Act of 16.02.2007 on competition and consumer protection (Journal of Laws of 2020, 1076 and 1086) with another economic operator who submitted a tender or a declaration of affiliation to the same capital group together with documents or information confirming preparation of a tender independently of another economic operator belonging to the same capital group. d) a copy or information from the National Court Register or Central Register and Information on Business Activity, within the scope of art. 109 par. 1 item 4 of the Act. 4 of the Act, made not earlier than 3 months before submitting it, if separate regulations require an entry in the register or register; - If the economic operator has its registered seat or place of residence outside the territory of the Republic of Poland, instead of a document as above, it submits a document or documents issued in the country where the economic operator has its registered seat or place of residence, confirming that it has not gone into liquidation, has not been declared bankrupt, its assets are not managed by a liquidator or a court, it has not entered into an arrangement with creditors, its business activity has not been suspended or is not in any other similar situation arising from a similar procedure provided for in the laws of the place where such procedure is initiated. - If in the country where the economic operator has its registered seat or where the person to whom the document refers resides, the documents referred to above are not issued, they shall be replaced with a document containing, respectively, a statement by the economic operator indicating the person or persons authorised to represent it, or a statement by the person to whom the document relates, made under false pretences, to whom the document refers, made under oath, or - if in the country where the economic operator has its registered seat or place of residence there are no provisions on declaration under oath - made before a judicial or administrative authority, a notary, a professional or economic self-government authority competent with regard to the economic operator's seat or place of residence. - Documents / statements referred to in a) and b) above should be issued no earlier than 3 months before the deadline for submission of tenders.
